Our organization is seeking qualified Psychiatric Nurse (RN ) to work with our geriatric population in their own homes. LOCATION: Genesee County Responsibilities The Journey Behavioral Home Health program, for patients, families, and caregivers, provides specific services and support tailored to the needs of individuals with mental illness, post-traumatic stress disorder, behavioral issues, and dementia, including Alzheimer's Disease and Parkinson's. Journey is an evidence-based, in-home behavioral health program, which meets all Medicare standards. The Psychiatric Nurse will be responsible for: 1. Performs case assessments and participates in the development and maintenance of treatment plans in accordance with The Journey Program. a) Develops, implements, and communicates patient plans of care. b) Completes nursing assessment forms in a timely manner. c) Conducts comprehensive biopsychosocial - spiritual assessments. d) Medication monitoring and administration. e) Collaborates with internal and external providers to coordinate psychiatric treatment with medical treatment. f) Participates in case conferences. g) Provides information regarding patients rights, staff assignments, and plans of care to patient and/or family members. h) Involves patients and family members in developing and implementing the plan of care whenever possible. 2. Implement current nursing practice according to the Psychiatric Mental Health Nursing Standards (ANA) . a) Assesses patient condition on admission and weekly thereafter as long as the patient remains in Agency care. b) Monitors clinical interventions for compliance with the defined plan of care. c) Analysis assessment data in determining a diagnosis. d) Develop a plan of care with input of the patient, other nurses, family and health care team. e) Identifies expected outcomes individualized to the patient. f) Implements and structures skilled nursing interventions to meet patient needs and changed conditions as identified by the plan of care. g) Provides, structures and maintains a therapeutic environment in collaboration with the patient and other health care providers. h) Employs strategies and interventions to promote and maintain mental health and prevent mental illness. 3. Responsible for acting as a case manager and communicating across disciplines to coordinate the patient's care a) For timely, accurate, and comprehensive documentation using the documentation required by The Medical Team for the Journey Behavioral Health Home Care Program. b) Communicates goals and service offerings to the community and provide public service/community educational programs. c) Responsible for acting in a caring, professional, and ethical manner for participating in the scheduled clinical meetings. 4. Participates in professional growth and development. a) Participates in regular educational programs. b) Demonstrates current knowledge of principle of basic nursing and psychiatric nursing practices. c) Demonstrates current knowledge of a coordinated interdisciplinary approach the provision of home health services. Qualifications and requirements Valid Registered Nurse License CPR/BLS Certification Minimum of two years of recent experience within the Behavioral Health field Ability to work independently as well as within a team environment Exceptional interpersonal & communication skills Outstanding organizational and self-management skills Proficient computer skills Must meet the minimum standards of Psychiatric Nurse as specified by Medicare Must pass competency exam with a score of 80% (Exam tests knowledge regarding medication management, characteristics of psychiatric diagnoses, therapeutic interventions, communication skills and supportive therapy techniques, and the ability to handle a psychiatric emergency) The Medical Team is an Equal Opportunity Employer.
