JOB DESCRIPTION: Director of Healthcare / Wellness Director (LPN) FULL TIME
JOB SUMMARY: The Director of Healthcare (DOH) will serve as the nursing clinical leader for The Landing of Bogart and The Landing of Winder. Both are 48-bed communities with a short 25- minute drive between each location. The DOH is responsible for the delivery of safe, evidence-based care to all residents. You will help train and supervise the clinical/direct care staff and set a high standard for how care is delivered ensuring all state/federal guidelines and community policies and procedures are followed. The DOH is responsible for overseeing clinical care to residents by coordinating with families, therapy, medical providers, hospice, pharmacy and other vendors. With support from other management staff, the DOH will help evaluate, design, implement and oversee quality improvement programs within the community. Each community employs an Executive Director who is also an LPN. The ED and DOH will work closely together to ensure quality care is delivered to our residents while supporting our team members.
" Meet with residents, families and team members to provide decision making for pre-admission, admission, and periodic resident assessments.
" Customize individual care plans (upon admissions and ongoing) for each resident.
" Utilize software to track resident activities, progression, and report findings to families and other team members.
" Provide clinical training and education to clinical care staff as needed.
" Provide full oversite of the medication management program to promote medication safety.
" Work with physicians, pharmacy partners, other clinical providers, and transportation to coordinate care and services for all residents.
" Collaborate with the consultant pharmacist on quarterly audit and reports, destruction of medication, and CMA licensure and continuing education requirements.
" Perform skills checks on team members passing medications and providing resident care to assure their capabilities.
" Ensure med carts are properly stored and maintain schedule of deliveries from the pharmacy partner.
" Assists in maintaining resident's health record including up-to-date weights, vital signs, and assessments.
" Serve as the infection control coordinator for the community.
" Coordinates with the pharmacy regarding medication refills, repacking, new resident's pharmacy enrollment and medication delivery.
" Assumes responsibility of the medication and medication carts and fills in with medication administration as needed.
" Oversees the certified medication aides ensuring proper procedures regarding medication administration, cart storage, and medication storage are followed.
" Develop, implement, and oversee policies and procedures related to memory care behavior management, care coordination, and care delivery to ensure that Alzheimer's or dementia patients are receiving passionate care in a safe environment that promotes safety and minimizes confusion.
" Develop, lead and maintain the clinical quality and process improvement initiatives within the community to mitigate risks and improve outcomes.
" Provide leadership and training by managing, directing, and monitoring all care staff to promote the best possible health and wellness delivery to our residents.
Ensure that resident showers, laundry and housekeeping tasks are completed by care staff.
" Research and stay abreast of findings related to improved memory care health delivery methods and work with the activities director and care staff to deliver improved care to the community.
" Encourage resident participation in programs by sending reminders, giving positive encouragement, and involving care team staff members.
" Ensure that all healthcare delivery policies and procedures are current with all state/federal regulations and are being followed by all team members.
" Ensure the clinical team properly documents and follows through with changes in clinical care needs.
" Collaborates with third party Health Service Agency and participates in interdisciplinary team meetings.
" Provides communication to necessary vendors and care providers in regards to needs within the community and as well as individual needs for each resident.
*Participate in the weekend rotation schedule with management to provide on-site weekend support 1:4 and provide "on-call" support in the event of an emergency or CMA shortage (administering medications).
" Order medical supplies/PPE and ensure clinical supply inventory is adequate.
" Assist residents with transitions as needed.
" Treat residents with respect, empathy and professionalism and help maintain their dignity through various situations.
" Provide on-call clinical support for management, team members, residents and family members.
" Responds promptly to resident requests and engages appropriate staff person if unable to fulfill resident needs/requests.
" Adheres to The Landing Senior Living Employee Handbook, the Employee Code of Conduct & all policy and procedures set forth by the company.
" Perform other projects or duties requested by management.
KNOWLEDGE, SKILLS, ABILITIES & JOB REQUIREMENTS:
" Graduate of an accredited school of nursing.
" Georgia LPN license with minimum five (5) years' experience in senior healthcare.
" Basic knowledge of and the ability to apply Geriatric Nursing principles.
" Strong communication and presentation skills.
" Ability to relate and work well with various individuals/personalities.
" Positive attitude and energy to coach and encourage resident participation.
" Completion of required training and maintenance of ongoing required training provided by the company and the State of Georgia.
" Ability to lead staff training or in-service when required.
" Ability to use good judgement and make decisions in the best interest of resident safety and well-being of the resident and the company.
" Ability to multitask and properly document all aspects of resident care delivery.
" Ability to delegate tasks to appropriate team members based on their skill capability and in compliance with regulations.
" Effectively use technology such as Microsoft Excel or Google Sheets, email, internet and eMAR.
" Experience with Point Click Care preferred but not required.
" CPR/First Aid Certification.
" Physical abilities demanded by the nursing profession to include aiding with patient transfers, administering treatments or medications, lifting/moving medical supplies, walking, standing, sitting for long periods of time and some car travel between locations.
" Valid driver's license and satisfactory motor vehicle history.
" TB Skin Test, PCR Covid Test, criminal background check and drug screen required.
