Position Responsibilities: • Receive report from off-going staff/give report to oncoming staff. (Including a discussion of each client, making notations of significant changes and new clients.) • Count controlled substances with the on-coming and off-going staff. · Note significant client information in the “Pass it On” book. • Read Pass On-Book since the last shift worked to keep updated on changes. · Attending daily staffing meetings. • Check appearance of any ill or injured client. Contact appropriate medical provider as needed. Provide treatment as ordered and document in medical record. • Administer routine and PRN medications and treatments as ordered. Document on the medication administration record/PRN sheet. • Obtain/record/review routine and as needed vital signs & weights. Notify Director of Nursing of significant health changes. • Document pertinent information on clients' medical record according to policies and procedures. • Assist with appointment set up when needed. Update Procentive shared calendar with appointments, intakes, and transportation needs. • Order medications, sign in medications from Pharmacy. • Write referrals/lab slips. • Take telephone orders from physician/NP and transcribe orders. • Order lab studies; report any abnormal values requiring immediate medical attention to the Director of Nursing. Send copies of lab values to physicians. • Complete incident reports and medication error reports when needed. • Answer the phone by the third ring and transfer calls to appropriate staff member or voice mail. • Notify Director of Nursing and Treatment Director of any emergency/disaster situations. • Notify dietary department of dietary orders and changes. • Maintain safety of the clients and security of the building. Report/correct actual or potential hazards. • Maintain med room and Tech Office in neat and orderly manner for up-coming shift. • Attend In-services and staff meetings. • Monitor the health services related tasks completed by Chemical Technicians. Keep Director of Nursing informed of any issues. • Respond appropriately to emergency situations. • Monthly MAR and Physician Orders review, setup, and obtain required signatures. • Health education to clients in group and staff in-services; under the supervision/delegation of the Director of Nursing. • File health paperwork and Procentive faxes into respective client charts; forward necessary paperwork to appropriate staff. • Maintain client health charts. • Any other duties as assigned by the Director of Nursing • Assist visitors/family/friends/physicians/lab/transportation personnel as needed. • Document physician, NP, psychiatrist orders after visits have been made. • Monitor level of med room supplies and notify Director of Nursing when an order is needed • Admit/discharge residents according to health services policies and procedures. ESSENTIAL JOB FUNCTIONS: At time of intake: 1. Height, Weight, and Vitals will be taken by LPN a. If Vital signs out of normal range, report readings immediately to RN. 2. LPN will be responsible for reviewing results of Urine Tests and Breathalyzers b. If breathalyzer test and/or rapid urine test (when applicable) is negative, proceed with admission c. If tests are positive for any substance notify RN for direction on how to proceed i. Report observed behavior ii. Report signs and symptoms of withdrawal or intoxication to RN iii. Report vital signs iv. Report date of last use as reported by the client. d. LPN will check results of lab urine tests and communicate results to RN and assigned counselor. 3. LPN will complete full Nursing Assessment form with client. Supervision: LPN will observe all health services tasks and documentation related to tasks and communicate with Director of Nursing when tasks and documentation are not within compliance with Health Services Policy and Procedures. Consultation: LPN will work directly with The Heights Director of Nursing regarding all delivery of medical care regarding compliance with medical policy and procedure, including medical referral, medication delivery to residents, and medical emergencies. LPN will work directly with the Clinical Director and/or The Heights Health Professional regarding compliance with Psychological, Psychiatric and other mental health services and referrals. The LPN will consult with outside providers as needed in regard to all Mental Health Issues at the direction of the Mental Health Professional. Follows resident plan of care: Utilize time efficiently to assure completion of duties and responsibilities in allotted time frame. Project sound judgment in delivery of care that is cost efficient, but does not jeopardize safe, effective care delivery. Follow company policies and procedures to ensure a safe, caring, comfortable, and clean environment. Utilize all appropriate, available resources for client, family, and responsible party. Update the physician and/or nurse practitioner, NP, as well as family/responsible party regarding the resident medical status. In an emergency, call 911 as appropriate. Network in collaboration with physician, nurse practitioner, and/or other health care professionals, regarding relevant information to changing plan of care. Question and seek clarification of unclear physician/NP orders. Delivers resident care: Deliver and maintain optimum client care and comfort by demonstrating knowledge and skills of current nursing practices. Document physician/NP orders appropriately, accurately and timely. Question and seek clarification of unclear physician/NP orders. Perform only those activities that are within the scope of acceptable LPN practice (takes orders directly from a registered nurse or physician, works directly with residents: administering medication, treatments, obtain vital signs, as well as clerical duties.) Notify the physician/NP if orders are not carried out and document the event appropriately. Follow company policies and procedures, and appropriately project sound judgment. Ensure accurate and complete documentation of all medical records and reporting forms (hard copy and computer based, if applicable). Promptly respond to client health needs. Monitor: Monitors certified Medication Passer's compliance with health services policy and procedures and accurate administration/documentation of medications. Alerts Director of Nursing of any concerns/issues with compliance of health services policy and procedures, and/or accurate administration/documentation of medications. Customer Service: Provide good, compassionate client care services. Establish and maintain positive interactions with internal and external customers, including co-workers. Noting that customers include clients, families, vendors and other employees. Resident’s Rights: Demonstrate strong ethical practice regarding individual/client rights as stated in the “Client Bill of Rights” and company policy. Demonstrate both sensitivity and appropriate responsiveness to each client’s wishes. Resident comfort and safety: Demonstrate safe practices in regard to client’s comfort and safety by applying knowledge of proper body mechanics for self and client, when assisting is needed. Practice and promote adherence to company safety policies and procedures. Pharmacy Knowledge: Maintain an updated base in pharmacology. Demonstrate knowledge of policies and procedures governing the six rights of medication administration. Demonstrate knowledge of drug reactions, sensitivities and nursing interventions. Assure that narcotics are accounted for properly. Apply adult learning principles when counseling individuals, family members or responsible party regarding the client’s medication regimen. Lab values: Recognize normal as well as abnormal lab values. Network with physician/NP and other interdisciplinary team members regarding lab values as appropriate. Infection Control: Demonstrate working knowledge of infection control policy, procedures and practices. Continuing Education/Staff Development: Recognize self-evaluation and self development as important tools in maintaining high standards of care and professionalism. Seek and provide peer support and consultation. Participate in and attend facility/nursing meetings and in-services as required. Utilize outside sources for required continuing education credits. Completed required co-occurring CEUs in required time frame after employment. Compliance: Must adhere to the Company’s Code of Conduct and Business Ethics policy, including documentation and reporting responsibilities.
