Job Description

Under general supervision and according to Food and Nutrition Services general policies and procedures, performs moderately complex duties including implementing the Friends from Meals on Wheels (“Friends”) Program. “Friends” is a senior socialization program aimed at decreasing loneliness and social isolation in homebound older adults receiving Meals on Wheels. The Program Coordinator will spend approximately 50% of time performing home visits, and telephone calls to clients and 50% of time performing program supervision and coordination.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Manage a client caseload of older adults and provide weekly home visits or phone calls to each client; or ensure that calls and visits are being made by a volunteer or intern. Calls and home visits may include: conversation, board games, reading and providing support to the older adult in accessing medical and other community resources.
  • With Manager supervision, oversee the “Friends” volunteer and intern programs, including: working with Community Partnerships manager to outreach for new volunteers and interns; orientation; monitoring and check-ins and evaluation of performance of volunteers and interns. 
  • Administer and oversee social isolation assessments and satisfaction surveys at regular intervals
  • Encourage participants to participate in the in-home exercise program
  • Facilitate and coordinate  group sessions, such as Televisits, for clients as requested
  • Coordinate the “Friends” Facebook page and regularly post appropriate materials
  • Oversee monthly newsletter production and distribution
  • Assign clients and activities to “Friends” staff, interns and volunteers in order to accomplish program goals.
  • Oversee outreach for and onboarding of new program participants.
  • Accurately track and record services in database.
  • Oversee the preparation of reports and informational handouts, brochures, flyers.
  • Run monthly, quarterly and annual reports to track outputs and outcomes; compare with program goals and grant requirements. 
  • Become informed about resources in Santa Clara County for older adults, and demonstrate the ability to effectively discuss them and to share information about how to access them. 
  • Participate in trainings, older adult forums, and partnership development. 
  • Uphold HIPAA client confidentiality.  
  • General office duties including answering the phone, copying, filing, word processing, and program record upkeep. 
  • Other duties as assigned. 

EXPERIENCE AND TRAINING:

  • Bachelor’s Degree preferred with major course in health sciences, social services, public administration, psychology, or a related field preferred
  • Minimum two years of progressively responsible administrative/office environment
  • Experience working with older adults preferred
  • Experience working with volunteers and/or interns preferred
  • Proficient in Microsoft Office products, Google applications 
  • Experience with databases such as Salesforce and Canva preferred
  • Bilingual English/Spanish or English/Vietnamese strongly preferred


MINIMUM KNOWLEDGE AND ABILITY:

  • Ability to build strong trusting relationships, and provide support to senior clients and their families through the use of: conversation, asking open-ended questions, empathizing, using motivational interviewing techniques, and/or recognizing changes in affect and behavior
  • Committed to improving the lives of seniors, and aware of health needs particular to the population of older adults
  • Excellent oral and written communication skills
  • Ability to work independently, prioritize and handle multiple tasks.
  • Ability to delegate tasks effectively to staff, interns and volunteers, and follow up with them
  • Ability to communicate effectively with culturally diverse populations
  • Highly organized and able to coordinate multiple daily activities
  • Ability to model and communicate the Friends from Meals on Wheels program and support a culture of empowerment and team support.
  • Knowledge of standard office equipment.

WORKING CONDITIONS:

  • About 50% of time in the field with clients; 50% professional office environment
  • Local travel required
  • Must have a car available for daily use, a valid California driver’s license and current automobile insurance
