DESCRIPTION of responsibilities:
Work with children, teens, adults, and families who are experiencing crisis
Provide positive intervention, stabilization, de-escalation, and conflict management using a trauma-informed and family centered approach.
Offer prevention, diversion, and intervention services
Document all services delivered to comply with agency, state, and federal regulations and requirements.
Attend agency staffing sessions, giving input into the decision making process regarding case management and goal setting for individual cases.
As necessary, supervise visits between children and their primary families.
Visit primary family, foster home, day care setting, school, and other appropriate settings to ensure that case management goals are being met.
Complete necessary paperwork to track progress and plan for achievement of established goals.
Evaluate case management services delivered to ensure services are meeting long-term objectives of the agency and meeting standards of time and cost efficiencies, as well as sensitivity to the cultural differences in the service population.
Attend court hearings, case staffing, or other activities necessary to coordinate client services.
Travel to deliver services to the family's home, and/or transport clients to alternative service locations.
Review and monitor advocate service provisions to ensure quality assurance.
Develop and deliver family centered casework services to clients participating in Youth, Family and Community Outreach Services Programs.

- Bachelor's Degree in Social Work or Human Service-Related Field (Required)
- 2 years of related work experience (Preferred)
- Must possess interpersonal, communication, time management, multitasking, computer, and teamwork skills.
- Must pass a DCFS background check
- Must be at least 21 years of age, obtain an insured vehicle, clean driving record, and three years driving record with a valid driver's license
