Works as a team member of all team to assess member needs and wants, participates in the development of individual service plans, acts as a member advocate and liaisons with families and community agencies.
Serves as the ACT Team s specialist for substance abuse issues by providing other team staff with information and knowledge in respect to substance use issues. Provides specific substance abuse interventions to clients as clinically indicated. Provides on-call crisis intervention services, as well as weekend and holiday coverage.ACT Substance Abuse Specialist duties include:Provides time-unlimited, community-based services to all clients served on the ACT team.Takes full responsibility for the treatment services for clients; coordinates care with other ACT team members and other providers to ensure client needs are met and that the client continues to receive treatment in the community to the greatest extent possible.Utilizes assertive engagement mechanisms, including street outreach and legal mechanisms (probation/parole, court ordered treatment), when necessary to re-engage clients in prescribed treatment.Is responsible for crisis services for all ACT team clients, coordinating hospital admissions and discharge planning.Provides all services to clients in accordance with established policies and procedures and clinical protocols to provide supportive counseling skill development and other support services as needed.Responsible for transporting or coordinating transportation for clients to necessary services, as outlined in their service plan, as needed.Documents all client events in the Electronic Health Record, in accordance with policies, procedures, and clinical protocols.Works with clinical team to assess client needs, develop individual treatment plan, provide direct care, coordinate services across providers, continuously evaluate client needs and functioning, and advocate for client needs in accordance with case management best practices and regulatory guidelines.Facilitates substance abuse supports, either individually or in groups, per clients individual service plan.Terros Health offers an excellent benefit package including, but not limited to: Medical, dental, and vision insurance Group life and disability insurance Employer matched 401(k) Generous PTO/paid sick leave (4+ weeks in year 1) Wellness and employee assistance plan$$ ACT incentives of an additional $2/hour$500 sign on bonus for new hires!Apply with your resume at www.terroshealth.orgEducation/Experience:GED/High school diploma with a minimum of 3 years of behavioral health experience or,An associatesdegree in the field of behavioral, orA bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, orA bachelor's degree in the field of behavioral, orA master's degree or higher in the field of behavioral healthBachelors Degree in the field of behavioralhealth or an equivalentcombination of experience and education Minimum of three (3) years experience in the delivery of substance abuse services requiredStrong understanding of substance use, addiction and recovery clinical skillsInterpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community membersMust have valid Arizona driver s license, be 21 years of age with minimum 3 years driving experience and meet requirements of Terros Health s driving policyMust have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employmentMust understand and utilize a recovery approach and philosophyExemplify great customer service and motivational skillsGood communication skills, evidenced by a non-judgmental approach, effective listening and positive interactionsMust be able to work on a computer and tablet in Microsoft Office, with electronic medical records and in NextGenProvide services in a clinical setting and in the community, as well as weekend and holiday coveragePhysical demands of this position are representative of those that must be met by an employee to successfully perform the essential functions of this job.
