Job Description

Tender Reflections Assisted Living & Memory Care, is accepting applications for a Community Director to oversee our Weston location.

The Community Director reports directly to the Regional Support Team of Sanctuary Care Group, LLC (SCG) and is responsible for overseeing the day-to-day operations of the assigned CBRF communities, including stabilized census and marketing, human relations, quality of care, DHS compliance, administration, resident billing support, policy and procedures, and maintaining resident's ISP's and other administrative duties.

Job Responsibilities

  • * Must be knowledgeable with DHS 83 regulations and to be able to enforce and maintain compliance *
  • Effectively train staff to policies/procedures and DHS 83 and audit to maintain compliance
  • Must have certifications in Standard Precautions, Fire Safety, First Aid & Choking, & Medication Administration
  • Must have prior Administrator/Management experience
  • Experience with Behavior Management w/Alzheimer's and Dementia
  • Perform prospective resident assessments and complete all necessary admission paperwork
  • Development/maintain of resident charts, care plans, & ISP's, medication orders with any resident change in condition or every 6 months and oversee their compliance
  • Audits of charts, care plans, & ISP's, medication orders, employee files for completion
  • Schedule and conduct ISP meetings with any change of condition and/or as scheduled
  • Effectively communicate changes in resident condition with resident's legal representative and/or Managed Care Organizations
  • Submit required supply orders, reports, and/or other documentation as requested by Operations
  • Submit required documentation to MCO's as requested
  • Basic understanding of infection control as it pertains to COVID 19 or any pandemic policies
  • "Manage by walking around"-rounds are completed at least three times per day to ensure standards and practices of Tender Reflections are being met
  • Administrative Rounding checking for compliance and safety checks
  • Oversee and ensure that all resident medication programs are carried out and adhered to
  • Oversee and ensure that all resident care information is properly logged in electronic documentation software and filed accordingly
  • Administer medication per Physician orders, DHS rules and regulations and nurse delegation
  • Develop and foster strong relations with resident families/representatives
  • Oversee and ensure that residents are engaged in daily activities through the life enrichment programming
  • Oversee and ensure that residents receive and participate in an effective and nutritious dining experience
  • Advocate on behalf of all residents
  • Complete monthly resident tracking
  • Oversee and ensure effective verbal and written communications
  • Oversee and ensure HIPPA compliance with all community personnel
  • Orient and train new employees, completing all paperwork timely and returning to Human Resources timely
  • Oversee and ensure all shifts are adequately staffed according to staffing calculator, trained and performing to respective expectations
  • Develop staff schedule, post, and store staff schedule in compliance with DHS regulations
  • May need to assist as a care provider to fill open shifts
  • Responsible for supervising, training, employee orientation, and staff meetings
  • Oversee and ensure compliance with overtime limitations
  • Document staff performance and complete all staff related documentation as required
  • Oversee cleaning schedules and ensure cleanliness of resident rooms and community
  • Seek SCG Controller approval and ordering of community supplies/equipment
  • Oversee emergency disaster plan training and implementation and conduct and or delegate drills as appropriate; ensure documentation as required per DHS
  • Maintain and document building inspection documentation (fire, sprinkler, etc.)
  • Maintain professional conduct and personal integrity in accordance with the Employee Handbook
  • Attend Strategic Planning sessions
  • Communication with Regional Facilities Manager (RFM) and Administration regarding building maintenance and equipment issues
  • Salaried position; 45+ hours/week, nights & weekends as needed / per on-call rotation
  • Attend in-house outreach events as coordinated by the marketing department
  • Develop, recommend, and foster relationships with third party providers
  • Support prospective resident tours as back-up to the marketing department
  • Perform additional job duties as assigned by Administration
