Qualifications

Education and Experience:

High School diploma and a minimum of one year experience in Human Services, related service or any equivalent combination of experience and training which provided the required knowledge, skills and abilities to serve those with intellectual disabilities. Must have a valid driver’s license. Certified in CPR and First Aide; behavioral management training required.

Knowledge, Skills, and Abilities:

Knowledge of mental illness and intellectual disabilities and must be able to distinguish between the two to ensure accurate services are provided. The ability to form effective relationships with other internal and external team members; ability to communicate effectively both orally and in writing.

Duties and Responsibilities:

Responsible for the support and training, included but not limited to, emotional support, physical support and monitoring, and development of life skills through training programs, all leading to increased independence and participation in community life for persons with developmental disabilities and/or other related disorders.

Provide education to teach the members about mental illness and appropriate medication to avoid complications and relapse, provide opportunities to learn and use independent living skills, and to enhance social and interpersonal skills within a supportive and normalizing program structure and environment according to licensing and Medicaid guidelines.

Ensure the participant’s homes are clean and well maintained. When participants are unable or unwilling to clean, it then becomes the responsibility of the staff to ensure the task is done. The staff will assist or personally complete household tasks.

Documents case activities for both clinical, collateral, and billing purposes.

· Coordinates with referral sources, case managers, psychiatrist, PCP, program

and other sources to link with external programs or services, including health care, recreational activities, transportation resources, financial and employment resources.

· Work with each participant on their individual finances and monthly budgets.

· Teach home and community safety skills.


Follow each participant’s s individual medication plan.

· Write incident reports and submit to the Program supervisor.

· Meet with program supervisor and client quarterly for the quarterly progress review meeting of members’ ISPs

· Meet with the program supervisor and client for any discharge meetings for members. Other duties as assigned by Program Supervisor, Clinical Director and Executive Director.
