Provides competent nursing care to clients based upon specialized knowledge and judgment derived from principles of biological and physiological sciences, in accordance with Tara Treatment Center’s Policies and Procedures.

• Assist in maintaining Tara Treatment Center, Inc.’s philosophy, goals, and objectives by applying and adapting these to direct client care
• Collects and interprets objective and subjective data (admission and ongoing)
• Identifies clients’ needs related to actual and/or potential health conditions and responds appropriately
• Participates with the multi-disciplinary treatment team to develop a treatment plan that includes nursing intervention when applicable
• Monitors, evaluates and documents client condition and response to treatment
• Alerts Physician, Nursing Supervisor and/or Clinical Supervisor to significant changes in the client’s condition
• Receives, transcribes and implements written and verbal Physician orders
• Maintains clear, concise, and timely documentation
• Administers intradermal, subcutaneous and intramuscular injections
• Monitors clients in the Detoxification program using OWS and/or CIWA assessments and protocol, implements standing Detoxification orders based on the outcome of the assessment and the clients vital signs
• Works with other members of nursing, clinical and administrative staff to promote optimal client care standards
• Evaluates present nursing practice and procedures and recommends changes when appropriate
• Completes admission process; nursing assessment, vital signs, breathalyzer, urine drug screen, nutritional screening, medication sheet, luggage and personal search and Detox Treatment Plan (if applicable)
• Observes and facilitates client self-administered medication, and utilizes established record keeping
• Provides client and/or family member’s education regarding nature of illness, treatment and medications
• Collaborates with client, family and multi-disciplinary treatment team in discharge planning when necessary
• Performs related work as required, which may include, but is not limited to: presenting educational material, participating in the on-call rotation, and attending all mandatory meetings/trainings
• Communicates in a confidential manner with other staff for the purpose of giving and receiving factual information
• Maintains a clean, safe environment and promotes positive client/family interaction
• Complete Incident Report form when applicable and route to Facility Administrator

• Current LPN/RN License in the State of Indiana
• Current Indiana Driver’s License
• Current CPR certification
• Annual Mantoux test or a bi-annual chest x-ray if applicable
• Must have a basic knowledge of the disease concept of alcohol/drug dependence and the 12-step programs
• Good verbal, writing and organizational skills
• Basic knowledge of database; internet and word processing software

• Monitor, evaluate and document clients conditions and responses to treatment
• Continually evaluate the appropriateness and necessity of nursing measures
• Gain clients trust and convey an atmosphere of safety in which the clients are able to discuss their concerns openly
• Communicate effectively with others
• Established ethics and professionalism
• Understands when it is necessary to alert the Nursing Supervisor, Clinical Supervisor or Medical Director to significant changes in client’s condition
