The Mental Health Specialist works in the Swords to Plowshares East Bay Service Center located in Oakland’s Jack London area providing case management services to veterans in a manner that exemplifies Swords to Plowshares philosophy, mission, and vision through direct veteran care. The Mental Health Specialist acts as a liaison between the agency and outside organizations, linking veterans with the appropriate resources to meet their needs.
Essential Responsibilities include but may not be limited to:
~Must be able to communicate effectively with a variety of personalities and be comfortable embracing the agency’s Diversity, Equity, and Inclusion Initiative and the principles of Cultural Humility.
· Ability to work with a diverse population- individuals experiencing co-occurring disorders, psychosis, suicidality, and other severe mental health presentations.
· Provides individual case management and crisis intervention.
· Conducts mental health assessments and develops goals and objectives in collaboration with veterans.
· Engages with veterans and their family when needed to ensure resources and referrals are provided for linkages to care.
· Conducts groups and activities designed to engage veterans and build community support systems.
· Ensures quality of services and clinical documentation follow Swords to Plowshares and the various funders policies, procedures, and standards.
· Professionalism, punctuality, flexibility, and reliability are imperative.
· Demonstrated ability to work collaboratively, with a team, in an effort to provide resources and services.
· Ability to participate in group decision making in addition to case conferencing with internal and external partners.
· Performs other duties as assigned.
Requirements include
· Master’s degree in a behavioral health field.
· Working with Veterans is desired but not required.
· Understanding of working with marginalized communities, people with disabling conditions, and persons experiencing homelessness.
· Ability to create and monitor progress towards identified goals.
· Experience engaging and assessing for suicidal ideation, prior suicide attempts, and safety planning.
· Timely clinical documentation
· Crisis intervention and De-escalation skills
· Excellent verbal, written, and communication skills.
· Strong organizational skills.
· Ability to follow through with tasks in a timely manner.
· Ability to work cooperatively and effectively as part of a team.
Equipment and Software used PC, Microsoft Word, Outlook for e-mail and calendar, SharePoint, web-based case management software, Homeless Management Information System (HMIS)
Supervisory responsibility: None
