Swope Health is seeking a full-time Behavioral Health Consultant (BHC). The primary responsibility of the BHC is to provide brief, targeted, real-time assessments and/or interventions during office visits to address the behavioral health needs of patients in primary care and to follow-up on all referrals from primary care providers. The BHC is an embedded, full-time member of the Primary Care Team. The BHC collaborates with other Swope Health departments and refers patients for care as needed.
Provides assessment, consultation, and brief intervention for behavioral health disorders.
Develops and implements, in collaboration with the Primary Care Team, an integrated, brief care plan and assists patients in successfully achieving goals.
Engages individuals and families as partners in services and develops positive, hopeful, and trusting relationships.
Monitors, evaluates, and records patient progress with respect to treatment goals and modifies care plans according to changes in client status.
Evaluates crisis situations and applies appropriate interventions.
Actively participates as a member of the interdisciplinary treatment team to support an integrated health care model that provides comprehensive care for patients. The BHC will provide clinical support and consultation to team members toward advancement of patient improvement in health outcomes.
Assists in the detection of “at risk” patients and in the development of plans to prevent further psychological or physical deterioration.
Assists the Primary Care Team in care management processes such as the use of guidelines, disease management techniques, behavior modification techniques and patient education to improve self-management of chronic disease.
Recognizes the role of the social determinants of health (SDOH) in achieving individual patient health goals for behavioral health disorders and chronic disease. Identifies SDOH, places appropriate referrals and incorporation into treatment/care plan.
Teaches patients, families, and staff care, prevention, and treatment enhancement techniques.
Monitors the behavioral health consultant program, identifying problems related to patient services and making recommendations for improvement.
Participates in performance improvement activities related to Behavioral Health Care and Primary Care.
Maintains productivity standards as established by program requirements and the organization.
Other duties as assigned.
Licensed Specialist Clinical Social Worker (LSCSW) required.
One to Three years of professional experience working with individuals experiencing behavioral disorders including mental illness and addiction.
Experience working with individuals in a community health setting preferred.
Proficient knowledge, utilization, and/or familiarity of the DSM-5/ICD-10, ability to assess mental health disorders, and make person centered treatment and service recommendations.
Proficient knowledge of health and human services and community resources.
Strong interpersonal skills, excellent verbal and written communication skills.
Strong consumer advocacy skills with an empowerment philosophy.
Ability to satisfy all clinical competencies as it relates to service delivery.
Training in Motivational Interviewing Techniques and Cognitive Behavioral Therapy; preferred.
Proficient with Microsoft Office Products including Word, Excel, Access, and PowerPoint.
