Job Description: Provides nursing care and service during the preoperative and postoperative periods; provides care for patients requiring a medical or surgical procedure that requires general anesthesia, deep or moderate sedation, local, and/or regional anesthetic; integrating physician orders and making nursing judgments pertaining to the patient’s condition on a routine and emergency basis, assessing relevant parameters of the patients' physical, psycho-social and spiritual status, following all established SCS, St. Vincent Anderson Regional Hospital, AORN, TJC and National Safety Guidelines. Oversees the supervision of the non-licensed staff. Performs call duties on a rotational basis and Preadmissions weekly assignments. Essential Duties and Responsibilities Preoperative Unit Responsibilities (Preop) · Prepares the patient for surgery according to physician orders. o Escort patient to the Preop Unit. o Reviews the patient’s chart. o Interview patient regarding their medical history/medications; applies armband and allergy bracelet. o Performs full body assessment on patient. o Initiates IV access; prepares and administers ordered IV medication and fluids. o Charts patient records according to St. Vincent Anderson Regional Hospital policy and procedures. o Performs lab draws as ordered. o Performs pregnancy tests as ordered. o Performs EKGs as ordered. o Schedules tests as ordered. · Performs Accuchecks on admission of all diabetic patients. · Organizes patient charts. · Alerts physician to any abnormal test results in Preop. · Facilitates patient flow in Preop. · Provides family and patient teaching and information/instruction regarding surgery and patient care. Preadmissions Responsibilities Accurately collects, identifies, and documents specific patient needs from pre-admission assessment process including: · Current medical diagnoses and therapies · Patient physical and physiological responses · Psychosocial status · Patient’s cultural, spiritual and lifestyle information · Patient’s understanding, perceptions, and expectations of the procedure · Patient’s response to past medical treatment, prior hospitalizations and surgical interventions · Verifies allergies both verbally with patient and with written documentation · Involves patient, significant others and other health care providers as required in the assessment. · Identifies obvious patient needs and sets priorities for care Procures necessary testing prior to the procedure as ordered by surgeon and anesthesia. Provides for discharge needs, initiates calls to Case Manager and Financial Counselor as needed. Communicates pertinent patient information to the Clinical Manager, physician, family and other health care professional involved in the patient’s care. Post-Anesthesia Care Unit (PACU) · Applies appropriate monitoring equipment to the patient. · Performs ABC assessment on every patient upon admission to PACU; assess for pain level, sedation level and monitoring the progression of vital signs back to baseline. · Monitors patient closely throughout progression from Phase I and into Phase II. · Prepares patient for discharge. · Provides information/instruction to patient and family members regarding discharge instructions. · Documents cardiac monitor and strips. · Provides thorough observation data and communicates pertinent patient information to the anesthesiologist/surgeon on an ongoing and timely basis. · Completes treatments and procedures as assigned based upon patient complexity and commensurate with the skill, knowledge and training of the individual. · Provides care safely, with adequate quality, quantity and timeliness and according to ASPAN standards and required documentation. · Reviews and initiates physician’s orders, administers medication, parenteral fluids and treatment as needed. · Performs and records the orders of physicians including administration of medications. · Monitors and assesses patient condition throughout postoperative period and reports any unusual symptoms, occurrences or discrepancies to Clinical Manager, anesthesiologist and/or surgeon. · Distributes educational material to patients and families as appropriate. · Able to provide conscious sedation monitoring/ACLS certification - ongoing assessment and monitoring of vital signs, EKG monitoring, pharmacology, and airway management. · Assist anesthesiologists with various blocks, epidurals, and procedures. · Uses and provides care for hemodynamic monitoring, such as arterial lines, swan gantz, CVP, etc., including obtaining and interpreting readings, assisting with insertion of, removal of, calibration, blood drawing, wedging, etc. · Performs neuro checks. · Performs vascular checks including use of Doppler. · Monitors and regulates vasoactive drugs. · Monitors body temperature and elevates body temperature as needed through use of warming devices. · Monitors surgical site. · Monitors all types of drains. · Maintains epidural catheter, along with epidural medication administration, catheter flushing, filter and cap change. · Maintains CADD pump including set-up, cassette change, bolus doses and documentation. · Maintains PCA pump including set-up, tubing changes and documentation. Core RN Responsibilities: · Follows all established SCS, St. Vincent Anderson Regional Hospital/Ascension Health System, AORN, TJC and National Safety Guidelines. · Maintains an accurate and legible patient medical record. · Maintains accurate record of supplies used. · Contacts patients within 24 hours post-op to answer questions and to ensure no complications related to the procedure. · Contacts patients prior to procedure with Preop instructions and arrival time. · Oversees the supervision of the non-licensed staff. · Responsible for assisting in maintaining a safe working environment throughout the organization by adhering to all safety management policies and procedures which reflect the mission and philosophy of the Ascension Health System. Attends all mandatory in- service programs and maintains professional and education requirements. · Supports patients' rights through advocacy, counseling, teaching, and inclusion of patient/family in decision- making. · Assists other staff members as requested or needed during active and waiting-for-work time periods. · Utilizes a cost-effective approach to care delivery. · Supports and promotes a professional work environment which includes shared decision-making and advancement of professional practice and personal growth. · Determine the physiological, emotional and developmental status of all patients across the life span undergoing a surgical procedure, taking into consideration age specific needs. Demonstrates age-related knowledge and skills necessary to provide care. · Problem-solves and gets resolution to barriers that hinder communication. · Completes all SEED Modules · Maintains ACLS, PALS and Healthcare Provider CPR. · Attends all mandatory in-service, training/education programs and development activities to improve job knowledge and skills, including mentoring, precepting, problem-solving, etc. · Attends all staff meetings (or reads meeting minutes). · Maintains professional and education certifications required for the job on a current basis. · Writes appropriate verbal/phone orders per physician orders. · Handles high turnover of patients. · Charts patient records according to St. Vincent Anderson Regional Hospital and AORN and ASPAN policy and procedures. · Uses supplies and equipment to achieve cost-effective care. · Demonstrates commitment to the organization’s mission. · Sets-up, operates and/or monitors the use of various systems, equipment and instrumentation required for the care of patients. · Maintains work areas and equipment in an aseptic and functional manner. · Delivers care for identified patient population and scope of service. · Demonstrates excellent critical thinking skills while providing excellent customer service behavior. · Practices principles of asepsis, sterilization and Bloodborne Pathogens and PPE usage. · Actively participates in department QAPI process and incorporates outcomes into own practice. · Able to provide clear communication handoffs utilizing the SBAR model when transferring care of patient to another RN. · Able to administer blood and blood products. · Transports patients as needed · Applies and manages patients with various splints or devices such as wound vac · Performs bedside blood glucose monitoring and necessary blood glucose meter quality control and maintenance · Participates in the orientation of new nursing staff as requested Company Description: Surgical Care Specialists is now owned by a group of over 20 physicians and other investors.
