JOB DESCRIPTION

Job Title/Position: Bi-Lingual Intake Specialist

Program: Referral, Intake, and Assessments

 

JOB SUMMARY

Provides front-line intake services to clients. This position is accountable for reviewing eligibility and service program expectations prior to clients’ active participation. Must possess a thorough understanding of the admission process and criteria. The individual is responsible for ensuring that clinical and administrative intake services are provided. Review the intake criteria and scope of services for the level of care recommendation with the individual/guardian. Conducts comprehensive face-to-face, or telemedicine-based Intake of clients. These functions serve as the basis for scheduling initial Comprehensive Diagnostic Assessments, Person-Centered Plan, or Treatment Plan, which includes a comprehensive crisis plan. This position is office-based.

 

REPORTS TO: Clinical Director (Assessments) – Clinical

QA/QI Director (Referrals/Intakes) - Administrative

 

DUTIES AND MAJOR RESPONSIBILITIES

· Receives and/or completes referrals in their entirety.

· Checks the electronic NC Tracks system to verify current Medicaid insurance coverage prior to notifying the potential client.

· Calls individuals to schedule an appointment for intake and assessments.

· Enters applicable information received onto the agency’s Referral Tracker.

· Reviews scheduling tracker for the availability of clinicians for conducting the assessment.

· Notifies clinician to conduct the assessment.

· Follow up with the individual to confirm the date and time of the scheduled assessment and update the agency’s Referral Tracker.

· Demonstrates professionalism by contacting the recipient in a timely manner when assessment appointment is changed; also, effectively follow-up and communicate with the recipient, when needed.

· Conducts intakes by explaining to the client and/or legally responsible person what each form represents; obtain appropriate signatures and dates.

· Completes Authorization to Disclose Health Information Form and fax to the entity(ies) to request coordination of care documentation.

· Develops and utilizes a tracking mechanism of all Authorization to Disclose Health Information Forms that are faxed to stay abreast of documentation received for service charts.

· Utilizes cultural competency skills accordingly and embraces cultural diversity while performing intakes. 

· Ensures signature and date and proper credentials are affixed on the assessment tool.

· Responsible for retrieving documents from the service chart and faxing to applicable agencies upon receiving a legitimate signed/dated disclosure.

· Generates service charts.

· Responsible for discharges in MCO’s provider portals.

· Checks the verification of Medical eligibility monthly.

· Maintains confidentiality regarding consumer information in a professional manner.

· Professionally communicates with outside entities when making referrals for appropriate levels of care.

· Participates in mandatory staff meetings facilitated by the agency.

· Participates in trainings necessary to perform the job efficiently.

· Audits service notes, Diagnostic Assessments, and PCPs as needed for scrutiny of quality assurance.

· Participates in required monthly supervisions.

. Ensures Client's charts are safe and can be easily retrieved. 

 

 

 

 

MINIMUM JOB REQUIREMENTS

· Experience conducting Intakes. 

· No substantial findings of abuse or neglect are listed on the North Carolina Health Care Personnel Registry.

· Effective written and verbal communication skills, as well as computer skills.

· Data entry skills.

· Superb customer services skills

· Knowledge of HIPAA requirements regarding client confidentiality
