This position is responsible for providing psychiatric rehabilitation services (PRS) to clients in the life domains of living, wellness, learning, working, and socializing. The Psychiatric Rehabilitation Worker will conduct initial and ongoing functional assessments, develop recovery-oriented rehabilitation plans with enrolled clients, and provide person-centered interventions according to each client's plan. Clients include youth/ young adults who are transitioning to adult roles; adults may also be served. The incumbent will provide input to the team regarding the client's progress, goal attainment and continued need for such services. The goal is to help individuals develop the skills that will allow them to engage in the community, develop meaningful roles, and have lives that bring them joy.
Bachelor's degree orHighSchool Diploma, GED or equivalentwith two years of Human Service experience including one year of direct mental health experience in a mental health related field. Effective communication skills, both written & verbal, organizational skills, knowledge of practical self-care skills & ability to work with minimal supervision. Ability to effectively work with persons with mental illness and/or developmental disabilities & implement rehabilitation plans for their program participation. Act 33/34 Clearance and FBI Criminal Background Check. Valid driver's license with access to a vehicle during scheduled work hours. Additional program specific minimum qualifications may apply.
