SKILLED NURSING FACILITY
Works in concert with the Executive Director and directs the Health Services and Health Services Support Departments to maintain quality standards of care in accordance with current Federal, State and organization standards, guidelines, and regulations. In absence of the Executive Director, assumes the responsibility for facility operations. The position conducts the nursing process – assessment, planning, implementation, and evaluation – under the scope of the State’s Nurse Practice Act of Registered Nurse licensure.
Currently licensed as RN in the state facility is located.
Associate's Degree in Nursing required; Bachelor’s Degree in Nursing preferred.
Persons who have been found guilty by a court of law or identified in by any registry, regulatory, or licensing body of abusing, neglecting, or mistreating individuals in a health care related setting are ineligible for employment in the position.
Five years in long-term or acute health care preferred.
At least 2 years nursing supervisory experience required.
Lifting 35 lbs. frequently and occasionally lifting 50 lbs. maximum. Talking, hearing, and seeing.
Knows and respects patients’ rights.
Ensures protected health information is kept confidential.
Ensures patient concerns/complaints are responded to with tact and urgency and reports such complaints to the Executive Director.
Reports allegations of patient abuse, neglect and/or misappropriation of patient property.
Safety and Sanitation
Follows established safety policies and procedures.
Observes safety needs of patient as indicated in care plan; participates in restraint reduction program.
Wears and/or uses safety equipment and supplies when indicated and properly trained to use.
Demonstrates job-specific knowledge of fire and disaster preparedness during drills or actual situation.
As required for regional or organization meetings and marketing duties.
Attends and participates in scheduled in-service training, educational classes, and meetings to maintain current certification as applicable and as managed by regulatory agencies and organization policies.
Participates in QAPI program using company approved tools.
Ensures that small group meetings are held per organization policy.
Communicates new Policy & Procedures and regulations to staff to ensure compliance.
Attends and participates in orientation classes to welcome staff, articulate expectations, and review company culture.
Quality Process Responsibilities
Role model for the company’s values and policies and collaborates with the Executive Director to ensure implementation and compliance.
Drives ongoing Quality Assurance process, oversees milestone plans for performance improvement, and monitors clinical trends and outcomes.
Uses TSI reports, CMS reports, current survey status and clinical outcomes to monitor clinical progress monthly.
Communicates care delivery trends, issues and opportunities to administration, medical director, and the Quality Assurance Committee.
Anticipates customers’ needs and works to minimize potential problems.
Participates in and promotes patients’ rights and customer service programs.
Ensures staff ability to effectively utilize the compliment/complaint process.
Supports, promotes, and consistently utilizes the Quality Assurance process.
Ensures ethical care delivery with adherence to corporate compliance and safe business practices.
Understands and promotes the Federal, State, and organization position on Abuse and Neglect prevention
Understands federal and state survey enforcement practice and to attain and maintain compliance; educates staff.
Collaborates with Medical Director to assure physician compliance with all federal, state and organization standards, policies, procedures, and guidelines.
Ensures staff comprehension and compliance with the documentation principles and guidelines.
Leadership Fundamental Responsibilities
Oversees the development of the nursing department structure and roles of each position.
Establishes and manages goals and objects for the nursing department through analysis and strategic planning consistent with the company mission.
Understands and affects the role of the nurse leader with staff through effective team building, motivation, change management, efficient time management, delegation, stress management and effective and timely communication.
Facilitates collegial relationships among nurse managers and other healthcare professionals.
Hires, educates, and counsels a nursing leadership team to participate in clinical oversight.
Maintains and evaluates an environment that empowers and supports nurse managers in problem solving and in decision-making to address identified issues.
Designs systems of share governance in clinical practice and accountability.
Ensures availability of nursing leadership team members to accommodate staff education; to address patient care issues; to meet with family members unavailable during routine shift hours.
Attends quality and operation reviews for the facility with Executive Director, Regional Clinical Consultant, and Regional Director of Operations.
Participates in meetings and conducts rounds with the Executive Director, Regional Clinical Consultant, and Regional Director of Operations.
Provides leadership to the facility in the absence of the Executive Director.
Clinical System Responsibilities
Promotes nursing process and critical thinking in nursing care delivery.
Institutes organizational structure and effective communication systems, which include all levels of nursing staff to promote effective care delivery and follow-up.
Oversees the consistency of clinical systems within and between clinical units and specialty areas.
Designs systems to plan, promote, develop, assess, interpret, validate, and evaluate the implementation of clinical programs, policies, procedures, and forms.
Participates in clinical risk identification, strategy planning, and risk reduction.
Makes daily rounds and spot checks documentation accuracy and completeness on new admissions, patients with change in condition, patients triggering occurrence investigations, and patients to be discharged.
Ensures compliance with the RAI, Medicare PPS, Medicaid, and Managed Care processes and are managed within the requirements for reimbursement, regulatory and accreditation requirements.
Champions the nursing process by analyzing and evaluating nursing programs and related services rendered to ensure quality patient care systems; mentors staff through the critical thinking process.
Ensures and evaluates systems to plan, promote, develop, assess, interpret, validate, and evaluate the implementation of clinical programs, policies, procedures, and forms.
Ensures and/or provides nursing support to facilitate an effective admission process, patient/family education and discharge planning.
Personnel Development Responsibilities
Ensures professional staff, hold current licenses in the state of practice; that non-licensed nursing personnel providing care delivery hold current certifications after the fourth month of hire; and all nursing personnel including purchased services or contracted services providing care have criminal background checks and appropriate TB screening.
Ensures effective departmental orientation and on-the-job training for new nursing employees.
Ensures personnel are adequately educated to care for acute events, chronic illnesses of the frail elderly, cognitive impairment, end of life, and hospice care. In some settings, post-acute services/programs may dictate additional specialized education and competencies.
Completes performance appraisals for all direct reports in accordance with the organization protocols; oversees completion and reviews performance appraisals of all nursing department personnel.
Ensures mandatory in-service are provided annually, including but not limited to abuse prevention, documentation guidelines, corporate compliance and safe business practices, blood borne pathogens, and exposure control plan.
Ensures staff is educated on the survey process and regulatory requirements.
Utilizes corporate, community and vendor resources to provide educational opportunities for nursing personnel.
Conducts staff meetings on all shifts at least monthly.
Demonstrates developmental action to encourage and prepare nursing staff for advanced practice responsibilities or nurse management roles.
Provides 24-hour “on call” service to the nursing facility in case of emergency.
Resource Utilization Responsibilities
Collaborates with the Executive Director, Health Services leadership and the Staffing Coordinator to structure roles, shift routines, permanent and contingency assignments for each unit and position in the nursing department.
Participates in hiring, counseling and/or terminating employees within the nursing department.
Uses master scheduling process to effectively manage daily staffing levels of licensed and nursing assistant personnel to meet patient needs within the budgeted PPD and state-specific staffing requirements.
Monitors and maintains adherence to protocols regarding attendance, time off requests, call-offs, and replacements; keeps the nursing staff informed of any changes that may affect employment status.
Collaborates with the Health Services leadership and Central Supply Coordinator to ensure the appropriate amounts of supplies and equipment are ordered and stocked to meet patient needs; ensures only approved vendors and products are used.
Monitors effectiveness of Medicare PPS, Medicaid, insurance, and ancillary supplies tracking and charge systems.
Fiscal Management Responsibilities
Collaborates with the Executive Director and Regional Director of Operations in preparing the annual nursing budget using corporate guidelines to ensure adequate resources for staffing, supplies and capital items needed for patient care.
Utilizes payroll reports in managing and monitoring the staffing of the nursing department.
Ensures adequacy of supplies and equipment for current need and projects future cost impact.
Oversees the operation of the nursing department in accordance with the approved budget through effective use of facility financial reports.
Using the available reports, monitors PPDs, trends outcomes, documents variances and communicates corrective action plans to Executive Director.
Assists in planning, coordinating, and conducting continuing education programs and special in-service training sessions relevant to the needs of department personnel.
Staffs the department with capable people.
Ensures that employees are adequately oriented and trained to perform their duties.
Reviews employee performance annually and makes recommendations for pay increases, promotions or performance improvement plans as deemed appropriate.
Establishes and maintains effective two-way communication to understand the needs and concerns of employees.
Recommends changes in policies and practices wherever employee needs are not being met.
Receives, investigates, and responds to employee complaints.
Maintains confidentiality of necessary information.
Participates in staff meetings, departmental head meetings and other facility meetings and sits on required committees.
Follows organization policies and procedures.
Thinks and acts calmly and logically to meet unusual occurrences of the job without being thrown off stride.
Performs any miscellaneous work assignments as may be required.
