Housing and Community Support Specialist (OPEN & PROMOTIONAL)

Provides clinical behavioral health case management and individualized support services for adults diagnosed with serious mental illness and/or co-occurring disorders to obtain housing and facilitate independent living skills, community integration and an improved quality of life. Functions include assistance with housing, access to and utilization of community resources, effective symptom management, independent living skills development and emotional support.

WHO MAY APPLY: This recruitment is open to all applicants meeting the minimum requirements. Priority consideration will be given to Spokane County employees in a position covered by the Master Contract submitting applications between 10/24/19 and 10/31/19. First consideration will be given to applicants working in CSHCD in a position covered by the 1553 Collective Bargaining Agreement (CBA); second consideration given to County-wide applicants in a position covered by the 1553 CBA; third consideration given to applicants covered by the Master Contract CBA; fourth consideration will be given to all other applicants.




TOTAL COMPENSATION: $43,246.38 - $58,355.09 annually. Total compensation is an estimate based upon base compensation, median retirement fund contributions, and current employer-paid health and related benefit contribution rates (including medical, dental, life insurance, long term disability, and leave). The actual total will vary depending upon each employee's enrollment choices.



Coordination with the Spokane County Regional Behavioral Health Organization's (SCRBHO) contracted behavioral health agencies' enrolled responsible primary behavioral health care providers.

Participate in intake interviews; assists in completion of intake documentation; completes individual treatment plan-recovery goals, per Department of Behavioral Health and Recovery (DBHR) standards; completes all other necessary forms.

Assess individuals for program eligibility and assists in gathering necessary documentation for housing programs.

Design person centered treatment plans based upon individual strengths; encourage and promote a strength-based environment to assist individuals in meeting their individual life goals.

Conduct housing searches with individuals who may have substantial barriers to independent living, to secure placement in safe and satisfactory housing; advocates to prospective property management; advocates with Northwest Fair Housing Alliance for individual rights if a person is being discriminated against when attempting to obtain housing.

Navigate complex HUD paperwork for access to housing for chronically homeless, low income, disabled individuals/families.

Orchestrate leasing signing, condition reports, and coordination of moving services; schedule and facilitate physical moves through contracted moving services; assist individual in establishing utility services.

Maintain stabilization and integration into the community once housing goals have been achieved.

Assist individuals in maintaining their housing arrangement and in resolving crisis situations; mediate between individuals and property management with tenancy problems as they arise, (e.g. lease violations, eviction notices, etc.).

Maintain knowledge of and regularly collaborate with local governmental agencies (e.g. Department of Social and Health Services, Department of Motor Vehicles, Social Security Administration, Immigration Authorities, HUD housing programs and Spokane Housing Authority).

Determine additional community resources necessary and/or helpful in assisting an individual in reaching their treatment plan goals in recovery and independent living.

Educate individuals on activities of daily living. Connect individuals to services in the community for basic health and dental services, vocational programs, legal representatives, money management including payee resources, food banks, shelters, low cost household items, clothing banks, Fair Housing, SNAP money management program, Responsible Renters class, etc.

Provide transportation for housing searches, attendance at various events and access to community resources.

Report all individual behavioral health issues to the SCRBHO's contracted behavioral health agency's enrolled responsible primary behavioral health care provider in a timely manner to troubleshoot and coordinate appropriate supportive services for individuals, to maintain residence in a least restrictive environment, to ensure safety of individual in the community.

Advise the enrolled responsible primary behavioral health care provider or the county Designated Mental Health Professional when appropriate, if an individual shows threats of violence, verbal harassment, self-mutilation or is not attending appointments or taking medically necessary prescribed medications.

Assess severity of threats, felony charges and possession of weapons for safety of individual and housing providers.

Assist with the Community Centralize Individual/Family intake process for placement of Permanent Supportive Housing, Transitional Housing or Rapid Rehousing programs.

Coordinate all Community Integration Program (CIP) social events, including preparation and completion of monthly calendar and weekly fliers.

Complete all necessary documentation, including charts, records of service, Daily Service Activity Logs (SAL), monthly progress reports, Raintree data information, etc. within the timelines established for service delivery and to meet SCRBHO and DBHR standards.

Maintain Federal and State laws on compliance, confidentiality, and security protocol.

Coordinate community donations, including arranging furniture pick up, etc.

Perform other duties as assigned.


TRAINING AND EXPERIENCE: Bachelor's degree in social work, human services, counseling, or other behavioral science or related field OR an Associate's degree with two years of experience in housing or community integration. Prefer experience working with individuals with complex life issues who have chronic or serious mental illness. 

LICENSE: Valid Driver's License at time of hire and for duration of employment; Department of Health (DOH) Agency Affiliated Counselors certification within 60 days of hire.

CSHCD Supportive Living Program positions will need to obtain a National Provider Identifier (NPI) within 30 days of hire and must be maintained throughout employment. May not be a State or Federal (OIG) and System for Award Management (SAM) excluded provider, in any capacity for the Medicare, Medicaid, and all Federal healthcare programs.
