The Quality and Performance Improvement Specialist assists in the implementation of the Quality & Performance Improvement Plan through the education and training of employees, through the support and facilitation of quality improvement projects, and through communication. The person in this position works primarily at South City Hospital.

JOB DUTIES AND RESPONSIBILITIES:

  1. Directs, coordinates and integrates continuous quality improvement activities as outlined by the Hospital's Performance Improvement Plan and the Hospital's commitment to improving care and quality.
  2. Provides organizational and facilitation support to newly forming and ongoing quality improvement teams.
  3. Assists department managers in developing and implementing departmental quality improvement plans.
  4. Oversees the documentation of quality improvement activities hospital-wide.
  5. Promotes compliance with (quality) standards established by TJC, CMS, and government agencies and other regulatory or standard-setting agencies; organizes and administers the preparation and follow-up for the Joint Commission on the Accreditation of Healthcare Organizations accreditation survey at South City Hospital.
  6. Oversees the maintenance of department records, reports, statistics, files, facilities, equipment, and supplies in accordance with established procedures.
  7. Develops and conducts ongoing concurrent and retrospective medical record reviews to comply with current regulatory and accreditation standards and promotes quality documentation through analysis of data and communication of the results, as assigned.
  8. Facilitates the Medical Staff Clinical Review Process; coordinates physician review sessions; develops reports and performs other related duties, as assigned.
  9. Works in collaboration with Risk Management, Infection Control, Service Excellence, Case Management, and others, to follow up on cases requiring physician review.
  10. Serves as the Quality Management representative for designated teams/committees, including ongoing Medical Record Review, TJC Preparation, and others as requested; participates in other hospital committees/teams as requested.
  11. Develops, implements, and enters data into automated information system and generates graphs and/or summary reports. Sets up automated system by adding a review focus. Uses a patient tracking file to generate patient lists for specific medical record review. Transfers review data into other formats for presentation (i.e., MIDAS to Excel, etc.); compiles statistical reports and other miscellaneous reports as required daily, weekly, monthly, and annually. Provides data analysis support for Infection Control, Quality Management, Risk Management, and Medical Staff Office.
  12. Coordinates meetings as assigned. Assists in preparing agendas, meeting packets and minutes, writes letters, etc. Follows up on items discussed at meetings.
  13. Assists in preparing action plans in response to JCAHO surveys, CMS surveys, State audits, and other regulatory bodies.
  14. Other duties as assigned.

JOB REQUISITES:

  1. A) EDUCATION AND TRAINING:

BS or BA degree in nursing or a health care related discipline

  1. B) LICENSING/REGISTRATION/CERTIFICATION:

Commensurate with education and training. Certification by National Association for Healthcare Quality desired.

  1. C) EXPERIENCE:

At least 3 years experience in healthcare with a strong understanding of hospital organization and department functions.

  1. D) SKILLS AND ABILITIES:
  • A working knowledge of quality improvement principles and methodologies utilized in health care.
  • A working knowledge of The Joint Commission and its processes and standards.
  • Commitment to the basic philosophy generally in the Performance Improvement plan and ability to embody this philosophy in the work place.
  • An affirming style of leadership in working with others.
  • Knowledge of adult education theory and practice and the ability to plan, organize and lead educational sessions.
  • Knowledge of PC technologies, including operating and application software.
  • Good listening, writing and oral communication skills.
  • Ability to assess and to explain data from a broad perspective.
  • Knowledge of and ability to apply statistical process control in a healthcare setting.
  • A working knowledge of group dynamics and skill in leading groups.
  • Working knowledge of and skill in using computers and software programs.
  • Effective presentation skills.

PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

Ability to move throughout the hospitals. Visual acuity for reviewing data and reports, observing process flow. Audiological acuity for monitoring group discussions and interactions. Verbal ability to effectively express ideas and views.
