Job Description: Position Summary: The home health care clinical supervising nurse provides home health services and education to clients and their families. Coordinates plan of cares by working with physicians to anticipate client needs. Serves as the RN member of a multidisciplinary team to meet specific plan of care goals. Manages a case load of clients under a general direction of management. Trains, directs, and provides clinical supervision for Home Health Aides. Coordinates closely with related disciplines to ensure professional service delivery. Keeps management well informed of client and staff activities. The Registered Nurse also provides services to clients in accordance with the State Nurse Practice Act, Agency Policy and accepted professional standards of practice. Additionally, they will provide care using services utilizing a comprehensive base of nursing theory and nursing process and communicate/document observations and assessments. General Job Duties: · All employees are responsible for a basic knowledge of the employee handbook, along with the mission and vision of the Organization, and must conduct themselves with behavior that supports these values. · All employees are responsible for making the most of their working time and reach maximum potential by being on time for appointments, checking voicemails/emails as appropriate and completing deadlines in a timely manner. · All employees are expected to escalate issues as necessary. · All employees must conduct themselves in a respectful and professional manner as outlined in the South Central Code of Conduct & Ethics Policy, as found in the handbook. · All employees will specifically be trained on their job duties, however; other mandatory trainings may be deemed appropriate by management for one or all members of the team. · All employees must occasionally collaborate with the leadership team to create, revise, and maintain accurate job descriptions. · All employees must assist with the general upkeep of the office, including maintaining a clean and orderly work space. Essential Job Functions: 1. Responsible for planning, coordinating and providing general nursing care for clients. 2. Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for home health nursing.Performs comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.Develops and/or follows and individual Plan of Care that incorporates analysis of data and current scientific findings.Provides skilled interventions with focus for achieving realistic client outcomes within a specific time period.Reports changes and information necessary to modify and update the care plan to reflect progress toward goals (outcomes). 3. Coordinates continuity of client care by communicating with other staff members. 4. Educates and informs clients and caregivers regarding medical procedures and care requirements. 5. Monitors client health, takes vital signs, and administers medications and IV applications. 6. Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice:Assessment skills as applied to the client, family/support system, and environment.Teaching skills according to the client’s needs and within the context of the client’s environment, value system, and physical/emotional status.Medication administrations. 7. Charts, records, and documents client status and all required information. 8. Contacts physicians regarding client conditions as needed. 9. Practices nursing with respect for individual, cultural, and spiritual differences. 10. Communicates effectively and in accordance with Agency requirements.Reports significant changes in client status to physician and other member of the team in a timely manner consistent with client needs.Maintains client records showing systematic assessment, planning intervention, and evaluation.Participates in team meetings and clarifies individualized reporting responsibilities with all new assignments.Documents legibly and according to Agency documentation guidelines and standards.Follows Agency guidelines for completion of forms, reflecting the orders and care plan. 11. Provides support to staff personnel and communicates concerns to management. 12. Participates in all activities relevant to the professional services furnished, including the development of qualifications and the assignment of personnel. 13. Monitors safety and infection control procedures.Demonstrates knowledge of safety/infection control practices by complying with established policies and procedures.Recognizes and responds appropriately to potentially unsafe situations.Demonstrates safe practice in the use of equipment.Assesses environmental safety and takes initiative to prevent accidents and promote safety.Notifies supervisor for need for review and/or training related to equipment, procedures, or safety/infection control practices. 14. Maintains confidentiality in all aspects of the job. 15. Participates in ongoing, professional self-development.Participates in all mandatory education per Agency guidelines.Identifies needs for personal professional growth.Demonstrates competency in areas of assignment or identifies the need for additional training.Participates in the Quality Assurance Performance Improvement meetings. 16. The RN will be available at all times, either in person or by phone, during operating hours or when on call. 17. Performs other duties as assigned. Cross Training Requirements: This position will be required to cross-train in different departments and may assist in those departments for up to 40 hours per week, when necessary. Position Requirements: The requirements listed below are representative of the knowledge skills and abilities required. Required Skills and Experience:Current Registered Nurse License in the State of Iowa and/or compact state licensed registered nurse.Minimum of one (1) year experience in home health care or related health field experience, and/or at least one (1) year of supervisory experience in home health care or related health field.Licensed driver with automobile insurance in accordance with the state and/or Agency.Current CPR certification. Desired Skills and Experience:Ability to provide leadership and supervision to home health aide/homemakers personnel.Demonstrates effective written and verbal communication skills.Strong interpersonal skills and the ability to follow instructions, and work as a member of a team.Ability to work with a wide diversity of clients.Demonstrated ability to read, write, and follow a written Plan of Care.Be dependable and be able to work independently. Language Requirements: The primary language of South Central is English. Excellent communication skills, as defined as the ability to:Actively listen for total comprehension,Ask questions that enhance the understanding of a certain topic, andRelay information and/or instruction in a descriptive and understandable fashion in both written and verbal forms to the interdisciplinary team, team members, patients and families, and others while working with South Central. Mathematical Requirements: Basic mathematical knowledge is required, such as subtraction, multiplication, and division skills to the general degree. Reasoning Ability Requirements: Professional-level reasoning abilities are necessary to process complex information and prioritize services. Physical Requirements:Regularly be called upon to lift or carry 10-50 lbs.Normal vision, as corrected.Normal hearing, as corrected.Good hand-eye coordination.Ability to bend, pivot, reach overhead, squat, kneel, and crouch daily.Ability to use protective equipment and take proper precautions and emergency measures.Must be able to sit for long periods of time, view a computer monitor, and type (up to 8 hours a day).Must be able to stand and walk for long periods of time.Must be able to sit for long periods of time in front of a computer screen and type. Travel Requirements: Daily travel within South Central’s service area is required. A good driving record, valid driver’s license, and proof of auto insurance is required. Overnight travel is not required. Nursing visits are primarily (but not limited to) Clarke, Decatur, Ringgold, Wayne, and Appanoose counties. FLSA Status: Non-Exempt: This will be a full-time, hourly position. Schedule Expectations: South Central’s normal hours of operation are from Monday through Friday, between 8:00 am and 4:30 pm, but the position’s schedule may vary. Regular on-call hours are required. As long as performance expectations are met, employees may flex his or her schedule within the range of business hours, subject to preapproval of one’s direct supervisor.
