Sequoia MHS is non-profit agency that services adults with mental illness and addictions. The two residential homes and one residential facility are part of a larger agency that has outpatient programming for children & families, adults, supported employment, integrated dual diagnosis and more. At Sequoia MHS we believe that people with mental health disabilities can live independently in the community. We provide services to achieve those goals.
Provided supportive services to adults with a diagnosed severe and persistent mental illness in the milieu of a residential home/facility. Supportive services include meal prep, cleaning, transportation to and from appointments and/or community integration outings.
Assists with administration of medications as ordered by a licensed medical professional.
Assists clients in activities of daily living (ADLs) necessary to reach their personal goals on their Service Plan.
Collaborates with all treatment team members regarding menu planning, shopping, preparation of meals and kitchen clean up.
Oversees house safety (fire drills, security of house at night, arbitration of disputes among residents, monitor and report maintenance safety issues, etc.)
Document all meaningful interactions with residents at the end of each shift according to policy and Oregon Administrative Rules.
Responsible for documenting all incidents prior to completion of shifts and notifying supervisor immediately of any critical incident.
Must attend all staff meetings.
Staff must be awake and alert at all times.
Staff will maintain professional boundaries and ethics at all times.
High School Diploma or equivalent
QMHA credential preferred
Must be 18 years of age or older
Caregiving experience or volunteer work within Social Services
Comfortable using computers including electronic health records
Salary: $13.00 /hour
