Santa Clara County KAS Services support youth and young adults ages 6-21 years of age and aims to address mental health symptoms that are impacting the youth's ability to successfully meet their life goals. Services built on the values and strengths of each youth and family, and are tailored to meet their unique and changing needs. The Care Coordinator provides strength-based, culturally competent services, including Child and Family Team facilitation, group leadership, and coordination of those providing services to families, including Support Counselors and Family Partners.

  • Coordinate the youth’s Child and Family Team and develop the individualized treatment plan based on the family’s strengths and needs, including safety planning.
  • Maintain regular contact with Child and Family Team members.
  • Provide culturally responsive, trauma-informed and family-centered services.
  • Assist the youth and the Child and Family Team to access needed resources and strength-based mental health services, social services, education services, and other supports and services.
  • Provide on-call crisis/emergency services as needed for youth and families who access the program’s 24 hour crisis phone line.
  • Discuss the provision and quality of activities actually provided with the youth and the CFT and ensure that the activities being provided are responsive to goals and needs.
  • Provide transportation for the youth to appointments or to access services as required.
  • Maintain all required documentation in a timely manner.
  • Attend regularly scheduled staff meetings.
  • Comply with all requirements of child abuse reporting, Medi-Cal compliance, confidentiality and HIPAA regulations in accordance with standards set by Seneca Family of Agencies.

  • Master’s Degree in social work, psychology, or counseling
  • Eligible to register with the California BBS
  • Experience working with children who are part of the child welfare system; experience working with children under age five a plus
  • Strong computer, communication, and multi-tasking skills
  • Own vehicle, California driver’s license and insurance.
  • Obtain fingerprint clearances with the DOJ, FBI and CACI, in addition to health screening, CPR certification, and other regulatory requirements

  • Monday- Friday
  • Flexible schedule required, with the ability to work some evening and weekend hours as needed
