Pathways Community Health Worker - Posting #20807
Full-Time position available immediately for a Pathways Community Health Worker; will travel throughout Grays Harbor County but will have an office in Aberdeen. This position requires the CHW to be able to drive to meet clients, flexibility, self-initiative, and the ability to manage time, resources, and client caseload efficiently. Under the direction of the Pathways Supervisor, this position works closely with all clients, community organizations, and other departments. This culturally and geographically connected individual serves as a link between underserved communities and existing community resources. Through home visiting, this individual assists clients in overcoming barriers to health, social services, education and employment.
