Looking for a part time Certified Nursing Assistant to work second shift 2:30pm-11:00pm. 40 hours per pay period. Rotating Holidays and weekends required. SUMMARY: Provides various nursing care in attending to the personal needs, support and health status of the Sister residents at Sacred Heart. Works within a team structure with all members of nursing by performing nursing related duties that directly affect the overall care of the Sisters served. Has the ability to relate effectively and sensitively to frail and cognitively impaired elders and an ability to establish a helping, trusting relationship with the Sisters. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. One (1) year related experience and/or training: or equivalent combination of education and experience. CERTIFICATES, LICENSES, REGISTRATIONS: Current unencumbered Certification as a Nursing Assistant in the State of Wisconsin. BASIC EXPECTATIONS: 1. Develops knowledge of School Sisters of St. Francis (SSSF) services. 2. Maintains a professional demeanor at all times. 7. Follows policies and procedures as stated in the SSSF Employee Handbook, along with department policy and procedures. 8. Upholds HIPPA and confidentiality of health and personal information of the Sisters served. 9. Communicates with the RN PM Supervisor and/or Charge Nurse regarding the Sisters status as needed. 10. Communicates with confidence and clarity and can relate to diverse types of people. 11. Demonstrates the ability to listen and seek to understand information presented and communicates information and ideas so others will understand. 12. Fosters a positive, supportive environment based on SSSF mission values. 13. Performs nursing care according to established standards of practice 14. Keeps work area clean, neat and in an orderly manner. 23. Maintains a well-groomed appearance. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. 1. Demonstrates willingness to participate in activities to improve department performance. 3. Takes responsibility to ensure the highest possible satisfaction of the Sisters, coworkers and guests. 4. Demonstrates a positive attitude when working with challenging situations. 5. Communicates effectively with SSSF coworkers as well as with Community Care staff regarding Sister needs. 6. Handles conflict positively, seeks guidance when needed. 7. Communicates concerns to the appropriate person with a focus on problem solving verses blaming. 8. Actively promotes cooperation and teamwork among peers. 9. Demonstrates ability to adjust to changes in work assignment; is flexible. 10. Demonstrates willingness to assist others when needed. 11. Demonstrates a welcoming atmosphere to new staff and guest staff. 12. Demonstrates being a supportive mentor to new staff. 13. Takes an active role in learning about changes; offering suggestions. 14. Accepts coaching on issues to improve current job skills or learn new ones. 15. Follows the Sister’s current care plan and communicates all concerns to the Nurse. 16. Provides personal cares including toileting, bathing /showering, foot care, eating assistance, grooming and oral cares. May provide simple treatment as directed. 17. Encourage residents to direct ADL’s (activities of daily living) and per self-care when possible. 18. Uses proper infection control and proper standard precautions at all times according to established standards. 19. Observes residents for emotional, behavioral and physical changes in condition including, but not limited to changes in mood, skin integrity, bowel and bladder status, food/fluid intake, speech, ambulation and transfer ability. Promptly reports any changes in condition to the Nurse. 20. Transports residents to the clinic, recreation program area, chapel and dining area. 21. Responds appropriately and in a timely manner to personal alarm devices. 22. Completes documentation according to department policy. 23. Accurately and promptly reports changes in condition to the Nurse. 24. Provides accurate and complete shift to shift report verbal and written. 25. Responds according to Policy and Procedures during emergency situations. 26. Accurately documents and communicates incidents and accidents. 27. Monitors environment to assure safety and addresses/reports issues as they arise. 28. Monitors environment to assure homelike atmosphere and addresses/reports issues as they arise. 29. Demonstrates correct use of equipment; sit to stand, sling lift, oxygen concentrator/refill system, bed and chair alarms, electric beds etc. 30. Demonstrates competency with Nursing Assistant skills. 31. Demonstrates ability to obtain accurate temperature, pulse, respiration and blood pressure and document accurately. 32. Performs duties as assigned by the SSSF Family Health Services Director and the RN PM Supervisor and/or Charge Nurse. 33. Utilizes supplies, time and equipment effectively to avoid damage or waste. 34. Completes assignments during allotted scheduled time. 35. Provides constructive feedback as requested to assist in identification of performance concerns for peers. 36. Adheres to safety requirements such as a gait belt and when using high risk equipment such as lift devices. PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job the employee is regularly required to stand and walk. The person frequently is required to use hand to finger, handle or feel; reach with hands and arms, stoop, kneel, crouch, or crawl; and talk or hear and must be able to hear and be able to perform a physical assessment using a stethoscope. Specific hearing abilities required by this position includes; hearing call lights and safety alarms. The employee is occasionally required to sit. The employee must frequently lift and or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. Must be able to follow emergency procedures. WORK ENVIRONMENT: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Heat and air are controlled by separate thermostats in individual rooms. There are potential for exposures to blood, body tissues and fluids and exposure to infectious diseases. There is frequent stress of working with periodic staff problems and sick residents, interaction with the resident who may be confused, irrational, agitated and quick to abrupt mood swings. Position varies in number of hours per week and is typically 2:30pm-11:00pm (subject to change). Workdays may be long and irregular. Weekend and holiday hours are also required.
