Job Description


Reporting to our corporate Chief Medical Officer, the Medical Director of Provider Training & Care Model Innovation will drive rigorous & standardized clinical model training development & delivery, the organization's culture of clinical excellence, and continual improvement & innovation of care model standards in a globally-capitated provider setting. This role will also support the design, evaluation, and scaling of existing and new program areas (e.g., patient home visits, virtual triage/urgent care) that improve patient engagement and outcomes. Partners with CMO and clinical leaders to define priorities, build and advance the relevant program areas, and execute on priorities to advance our comprehensive care model and tangibly improve clinical outcomes across our national footprint.


  • Works with CMO to develop new clinician onboarding and training curriculum designed for our globally-capitated model

  • Creates & delivers clinical training content to providers

  • Supports onboarding and development of Primary Care Physicians and Center Medical Directors in their initial 90 days and as-needed on a continual basis

  • Participates and acts as a coach for facilitated practice, Clinician's meetings, and weekly huddles as required to ensure the best patient outcomes and team efficiency

  • Provides timely and actionable feedback to Clinicians by leading facilitated practice, role models exemplary clinical leadership

  • Drives strong mentoring and coaching culture

  • Drives a culture of engagement through Center Leadership and recommends action addressing opportunities.

  • Collaborates with Center Leadership to develop an exceptional workplace environment

  • Identifies opportunities with provider turnover and develops action planning steps in partnership with market leadership.

  • Cultivates a uniform clinical culture, clinical values, clinical approach, incentives, and performance values for our providers across all centers

  • Supports regional candidate screening and hiring efforts of new Clinicians, gains partnerships with local residency programs and other external community contacts to drive recruitment efforts

  • Identifies developmental gaps and recommendations to enhance performance

  • Supports training and coaching toward Sage Health best practices for new centers within the region to ensure a successful launch

  • Builds a strong relationship and collaborates with the VP of Operations to ensure training completion, adherence to best practices, and development of Clinicians

  • Drives a positive and welcoming approach to patient care, particularly for underserved seniors with low to moderate incomes


  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required

  • Board certification in a major primary care specialty field of practice

  • Current, valid license to practice medicine, including a DEA license

  • Strongly prefer a minimum of three (3) years of experience in progressively responsible administrative or management-related positions within a primary healthcare environment, preferably in medically needy and underserved communities and in a community health center setting.

  • With a demonstrated record of success, previous experience supervising providers and other clinical staff is essential.

  • Strongly prefer two (2) years of previous experience as a Medical Director or Associate Medical Director or equivalent with a Medicare or Medicaid patient population.

  • Previous experience providing services and exercising leadership in a culturally and linguistically diverse setting, with demonstrated success

  • Previous experience working within a managed care environment is preferred, especially with experience in administrative/management capacity.

  • Demonstrated ability to work effectively independently and as part of a team in collaborative settings

  • Demonstrated ability to provide leadership to staff and to build the trust and respect of patients, staff, colleagues, and external contacts

  • Able to motivate staff; influence people and outcomes, promote team building; assume a leadership role within the clinical arena.

  • Ability to select, train, and develop Clinical talent to maintain best center performance and patient outcomes

  • Excellent problem-solving skills required, including creativity, resourcefulness, timeliness, and technical knowledge related to analyzing and resolving medical/administrative problems.

  • Sensitivity to needs of culturally and linguistically diverse patient and employee population

  • Excellent emotional coping skills, adequate to confront difficult emotional situations, and emotional responses to others

  • Proficient in Microsoft Office Suite products, including Word, Excel, PowerPoint, and Outlook

  • Required travel 20%. National travel as requested.


  • Knowledge of HEDIS/STARS

  • Knowledge of CAHPS and HOS

  • Experience working in medically underserved/culturally diverse communities

  • Experience working with an electronic health record

  • Experience working in Medicare Advantage Global Risk Models

  • Experienced in motivational interviewing
