This position will provide CPST services/interventions to adults, age 18 and over, who are diagnosed with a severe and persistent mental illness. Responsibilities include, but are not limited to, providing case management services, providing coordination and ongoing assessment of needs of the client by developing and monitoring overall service delivery, providing intervention and collaborate with therapists, medical staff and various community agencies to assist client in reaching their mental health goals. Full and part time opportunities available.
Requires an Associates with 3 years working experience or a Bachelor’s degree in a mental health related field. Must be able to demonstrate time-management skills, ability to work independently in the community and the ability to work with a larger team. Access to an operational vehicle, automobile insurance ($100,000/$300,000 liability coverage) and a valid driver’s license is required. Must be able to pass a background check, motor vehicle check and drug test. Must maintain a current NPI number.
