ACT - Counselor

QUALIFICATIONS;

1. Personal stability, flexibility, energy and commitment to working with the chronically mentally ill.
2. Current CPR and First Aid certification; driver's license and safe driving record; reliable transportation.
3. Excellent verbal and written communication skills.
4. Bachelor's degree in the Human Services field and/or four years' experience working with chronically mentally ill clients.
5. Experience in substance abuse counseling is required.
6. Meet credentialing and privileging requirements as set out in Policy and Procedures.

DUTIES:

Using the electronic medical records, input data and following through seeing services to their completion.

1. Maintain total confidentiality and adhere to HIPAA law, CARF accreditation standards and Maryland State Regulations.
2. Take part in agency sponsored training programs by supervisor's request.
3. Assume primary responsibility for a small caseload, generate member assessments, develop individual rehabilitation plans and document progress towards achieving goals; other related documentation requirements.
4. Carrying out assignments to fulfill the ACT mission of providing sanctuary and support to members in the community including but not limited to: providing specialty expertise to the ACT team in substance abuse; regular community visitation; face to face and telephone contacts in accordance with ACT Team's schedule; housing assignments to teach independent living skills in the areas of shelter, food, clothing, medications, money, and ADLs; assisting members to find and maintain housing; job placement and employment counseling; training in accessing community resources; transportation; positive role modeling; teaching basic standards of cleanliness and safety; member assessment and crisis interventions.
5. Participate in agency employee meetings and ACT team meetings as requested.
6. Take part in a rotation of on-call for the ACT caseload evenings and weekends as scheduled.
7. Ensure that all programmatic and physical environment issues are in accord with agency policies and administrative regulations as appropriate.
8. Assist in coordination of services received from other community agencies.
9. Other duties as assigned by the ACT Team Leader.

NARRATIVE: The ACT Team will be the center for information, documentation, decision-making, and implementation of plans for individual members on a caseload. It will be responsible for the goals and strategies that make up the core of an individual member's program and progress. The ACT team

Addictions Specialist will share the responsibility to carry out the activities designed to assure ACT mission success on and off site, in groups and one-to-one contacts, in formal as well as informal situations. The ACT Addictions Specialist will have primary responsibility for certain members charts and treatment plans, shared responsibility for the ACT caseload, and shared responsibility to work out schedules, coverage, preparation, and implementation of all activities through a commitment to consensus building and teamwork primacy.

AUTHORITY: Position reports to Director of ACT (Lower or Mid-Shore Region) and receives schedule from same.

CLASSIFICATION, SALARY, AND BENEFITS: Regular, full-time position; exempt from overtime.
