POSITION SUMMARY: Under the direction of the Chief Medical Officer, the Medical Director, Adult Medicine, and Director of Clinical Programs (MD/DO) is the lead clinician in charge of all aspects of adult medical patient care services provided by Roots Community Health Center. This position is responsible for ensuring that our approach to delivering “Whole Health” to our community is embedded within all aspects of the clinic, including facilitating integration of outreach, navigation and empowerment services. The Medical Director monitors clinical performance to ensure adult medical services, programs and operations are in compliance with all applicable regulatory, licensing and funding agencies. The Medical Director is responsible for implementing, reviewing and developing Clinical protocols, performance objectives, productivity benchmarks, compliance measures practices, and prioritizing and troubleshooting medical services/operations issues. As Director of Clinical programs, oversees complex care, communicable diseases, and other adult medicine programs responsive to community needs. The Medical Director will also represent Roots at required meetings with affiliated agencies. As a licensed physician, the Medical Director will provide direct patient services in addition to their administrative duties. Duties And Responsibilities: Administrative Thought partner to CMO/CEO and key member of organization’s Management Team Support development and maintenance of a strategic plan for the controlled growth, expansion, and/or development of adult medicine programs and sites for Roots Community Health Center. Attends management meetings with the overriding and ongoing goal of integrating clinical and non-clinical aspects of Roots operations and services. In conjunction with the CMO, performs due diligence in matters related to clinical practice in full compliance of the medical licensing board, the CA business and professions code, and other regulatory agencies. Participates in recruitment of qualified clinical staff including interviewing and recommendation for hire. Monitors and assists with the new-hire orientation process to ensure consistency in training among all clinical providers. Ensures adult medicine programs, services and operations are in compliance with all applicable regulatory and licensing agencies. Assesses all matters of clinical compliance and provides timely recommendations for corrective action and quality improvement. Provides guidance and supervision of clinical laboratory operations to ensure compliance with CLIA and other agency requirements. Establishes and/or maintains working and collaborative relationships in the health provider community, on behalf of and in accordance with formal understandings of Roots Community Health Center. Develops relationships with hospitals and specialty providers for referrals and for provision of inpatient care Works closely with CMO to provide direction regarding all clinical issues related to managed care. Provides direct clinical medical services in the area of board certified (or board eligible) medical specialty in accordance with the highest applicable standards of medical and professional practice and in full accordance with health center protocols and policies. Provides additional assistance within and outside of clinical arena, as requested and/or as appropriate, to ensure the ongoing success of the organization. Other duties as assigned Programmatic Supervise clinical program staff Ensure all adult clinical program activities are conducted in a timely and effective manner and meet compliance requirements Conduct clinical program meetings and timely follow-up activities Ensures timely and accurate data entry and submission to management for review prior to external transmission, where applicable Essential Functions Perform the primary functions of assessment, planning, facilitation and advocacy through collaboration with clinic members and other health care resources involved in the member’s care. Work closely with licensed and unlicensed staff to co-manage the care of complex cases telephonically through regular contact with members, caretakers, healthcare professionals and others involved in the member’s care The nurse will act a subject matter expert on the nursing process and perspective for the team Participate in the ongoing process of identifying members who are most at risk of poor health outcomes and in need of care management services or other resources Assess member medical and social determinants of health and coordinate referrals to needed resources Communicate with providers, members, and community resources as necessary, to support the planning, implementation and evaluation of clinical program(s). Employ a patient advocacy approach with a seamless integration of services Assist in the development, implementation, and ongoing refinement of care coordination and referral policies and procedures in collaboration with leadership Implement a trauma-informed and compassionate care approach to program delivery from both a patient care and staff wellness perspective. Ensure compliance with HIPAA and policies and procedures for client consent and information sharing in program delivery and support staff in this regard Participate in community, agency and department committees and meetings. Clinical Supervise adult medicine physician, midlevel provider, nursing and medical assistant staff Provide leadership, direction, oversight and training as needed for clinical team Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury, according to the highest standard of care Order, perform, and interpret tests and analyze records, reports, and examination information to diagnose patients’ condition(s) Provide culturally responsive and situationally appropriate services Encourage patient self-empowerment, and the building of skills of self-advocacy and self-efficacy Collect, record, and maintain patient information (medical history, reports, and examination results, etc.) Monitor patients' conditions and progress and reevaluate treatments as necessary Explain procedures and discuss test results or prescribed treatments with patients Refer patients to medical specialists, wraparound services, and other practitioners when necessary Advise patients and community members concerning nutrition, lifestyle, coping skills and risk reduction Coordinate with nurses, social workers, pharmacists, psychologists, and other health care providers Direct and coordinate activities of clinical staff, students/externs and support staff. Perform duties and procedures within scope of clinical license and scope of primary care Plan, implement, and administer health programs or standards in clinic and the community for prevention or treatment of injury or illness. Meeting performance targets set by the organization for selecting treatment protocols Maintaining confidentiality and impartiality Compassion and a good bedside manner Ability to solve problems and make and communicate decisions effectively Provide patient education and support as needed Qualifications: Required Licensed M.D. or D.O. from a recognized University Successfully completed residency in Family Practice, Internal Medicine or Emergency Medicine Current, valid license to practice medicine in the State of California, including DEA in good standing Culturally competent working with African/African Ancestry patients and families Demonstrated ability to work effectively independently and as part of an interdisciplinary team, in collaborative settings Excellent written and verbal communication skills Understanding of/commitment to maintaining highest level of confidentiality Experience working with Electronic Health Records; Ability to keep accurate and clear records Able to motivate staff; promote team building; assume leadership role within clinical arena Excellent problem-solving skills required, including creativity, resourcefulness, timeliness, and technical knowledge related to analyzing and resolving medical/administrative problems Ability to interact effectively and in a supportive manner with persons of all backgrounds Willing to train on health coaching, hepatitis C treatment, medication assisted treatment of opioid use disorder, and other beneficial/emerging topics Preferred Two to five years’ experience working in community health or primary care clinic setting Experience with treatment of HIV, Hepatitis C and opioid use disorder Experience in a primary health care environment that serves medically needy and underserved, Medi-Cal populations with cultural proficiency working among African/African Ancestry patients Board certification preferred. Board eligible acceptable. Current, relevant knowledge of areas related to community health center licensing and operations, e.g., legal/regulatory, administrative, policy, protocols, and other clinical issues, especially within a managed care environment Knowledge and understanding of mid-level supervision agreements and regulations. Preferred proficiency in working with an urban underserved population, with individuals recently released from incarceration, homeless and other marginalized populations
