Job Description

We are seeking a Supportive Living Specialist (QMAP REQUIRED) to join our team! We work with adult survivors of traumatic brain injuries. Our homes are about family and self empowerment. It is our goal to make sure our individual residents' lives are celebrated and enjoyed. We are looking for a caring, loving, person who wants to spend quality time with our clients and help make their lives worthwhile and meaningful.

For example we are looking for someone who will be having meals with the residents, helping them plan trips and outings, watch a tv show together, sing some karaoke, go to the mountains, support their hobbies and activities, help with medical appointments, help them volunteer, help them take pictures and document their lives the way so many of us do with our friends and families. Provide emotional support as well as education and information about their lives and medical care.

We are a team at RMCC, the Executive Director is there for you daily, and in live time. She oversees all medical coordination for residents, as well as direct support for residents. We want those who work with us to feel supported in our mission and prepared to the best they can for our residents. This is their house, it is our job to make it happy, healthy, and homey.

RMMC Mission: To provide long term community based habilitative residential services to individuals diagnosed with a traumatic brain injury by supporting the growth of maximum independence in the least restrictive environment.

RMCC Residential Supported Living Program is designed for individuals who may have completed a post-acute or neurobehavioral rehabilitation program or are living alone or with family and need a more structured living environment to support their level of functioning.

RMCC offers community based residential services in the greater Denver area for adults. Our homes are located in wonderful neighborhoods, near open space, trails, and parks. Services and activities are convenient for our residents, allowing easy access to their community. We provide the resources and opportunities for each resident to live a diverse, full life.

RMCC residents benefit from:

24-hour Protective Oversight, Private bedroom and bathroom or a shared bedroom and bathroom (max 2 persons per room), Case Management and Advocacy, Community Integration and Socialization, Spiritual and Functional Behavioral Activities
Habilitation, Assistance in referrals, transport and oversight of any PT, OT, Speech, Voc Rehab, Cognitive and Behavioral Coaching services

Responsibilities:

  • Provide direct patient care and companionship
  • Administer prescribed medications to patients per QMAP regulation
  • Assist in the transport of patients
  • Daily Cooking/Baking from scratch healthy meals
  • Maintain a clean and healthy environment

Qualifications:

  • Previous experience in nursing, patient care, or other related fields
  • Minimum of two references, not of of family relation
  • Upbeat, positive, compassionate and caring demeanor
  • Familiarity with small assisted living facilities,
  • Ability to work well in teams
