Description/Job SummaryThe Program Specialist/Lifesharing Specialist is responsible for ensursing programmatic regulatory compliance and documentation. Creating an environment that empowers individuals who receive or provide support, through the development of skills and coordination of resources, in accordance with Everyday Lives and Person-Centered Practices.*Responsibilities/Duties** Oversee the quality of programmatic plans, ensuring individuals' health and safety is upheld in accordance with each ISP.Ensure that identified services, goals, and outcomes are accurately documented, adhering to PA Chapter 6100 regulations.Represent program in a professional manner; working in conjunction with Supports Coordination Organizations, Administrative Entities, the Office of Developmental Programs, and other ancillary services.Organize and maintain individual files in accordance with PA Chapter 6500 regulations and program requirements.Complete individual assessments, SEEPs, quarterlies, and other applicable documentation, within regulatory standards.Assist and prepare new Lifesharing Providers for the self-licensing assessment.Provide and conduct applicable trainings for Lifesharing families and direct care professionals.Complete visits to sites monthly, or as needed.Maintain a minimum of 24-hours of professional and service-related trainings per training year.Participate in supervision and professional development plans with program management.Serve as an incident management point person for the program within EIM.Participate in a regular on-call rotation with other team members.Required ExperienceAt least one year case management experience in supporting individuals in a lifesharing program, supportive living, community based residential program with a focus on person centered planning.Required EducationMaster's Degree from an accredited educational program with 1 years' work experience working directly with persons with an intellectual disability or autism, orBachelor's Degree from an accredited educational program with 2 years' work experience working directly with persons with an intellectual disability or autism, orAssociate Degree from an accredited educational program with 4 years' years' work experience working directly with persons with an intellectual disability or autism, orRequired QualificationsDemonstrate a strong, working knowledge of DPW Chapters 6500 and 6100 Regulations. Demonstrate strong professional, organizational, and leadership skills. Demonstrate the ability to work independently with a minimum of supervision. Ability to effectively communicate both verbally and in written form. Ability to support and implement RHD's Values, as well as the philosophy of Lifesharing. Ability to engage individuals, Lifesharing providers, and Direct Support Professionals in effective, therapeutic, and healthy interactions. Ability to utilize Basic Computer Skills and navigate Microsoft Office programs and applications.
