Job Description
Essential Functions:
  • Coordinate house-wide organizational response to ongoing and newly released TJC National Patient Safety Goals, Leapfrog, NQF, AHRQ and other related regulatory or community standard patient safety metrics. Partner with key stakeholders and the Executive Team for effective implementation throughout organization.
  • Survey, collect, assess and analyze organization experience data on errors/events, near misses and medical and hospital staff feedback/ideas to identify patient safety culture and tactical priorities for improvement.
  • Assemble, coordinate, record and facilitate multi-disciplinary teams to conduct gap and failure mode and effect analyses (FMEA) in response to Sentinel Event Alerts and organization-identified FMEA annual priority.
  • Facilitates Executive Staff and other patient safety rounds and response plans, conducts education for caregivers and physicians, on patient safety culture and best practices as well as support them in user-friendly application.
  • Participate in collaborative practice teams as applicable, both, internal and external, related to implementation of best practices in patient safety.
  • Partners with other members of Quality and Performance Improvement and Risk Manager Departments to assure integration of efforts along the continuum of Patient Safety to Risk and Peer Review.
  • Facilitates annual evaluation and summary report for Patient Safety program.
  • Provision of summary presentation reports to Perfect Care Quality Committee, Quality Operations Committee, Medical Staff Executive Committee. Oversight Committee, and other committees on patient safety, with recommendations for modification or intervention as appropriate.
  • Strong interpersonal skills to work with all levels of employees and physicians.
  • Demonstrates excellent verbal, written and presentation communication skills.

  • Ability to maximize output to multiple deadlines, while maintaining a calm demeanor.
  • Strong facilitation and complex problem-solving skills.
  • Ability to review and analyze patient safety-related events and data, independently.
  • Demonstrated computer skills.
  • Working knowledge of Microsoft Word, Excel and PowerPoint.
Minimum Position Requirements:
Education: Bachelor's Degree
  • 5 years Direct patient care experience in acute care setting.
  • Experience developing and leading patient safety program.
Preferred Position Qualifications:
  • Master's Degree in Healthcare field.
  • Coursework/Training - Training in performance improvement, data analyses and safety tools and techniques, as well as team dynamics and facilitation skills.
Experience: Exposure to quality management field.
Licenses/Certifications: CPHQ or CPPS.
