October Road is looking for a Nurse to join our Assertive Community Treatment Team! This candidate will be Responsible for the following: + Participate in regularly scheduled treatment team meetings + Provide clinical expertise and interact with multi-disciplinary treatment team + Maintain regular contact with referral sources as indicated + Develop appropriate discharge plans collaborating with referral sources + Monitors Medication compliance, adverse effects + Gives IM injections per prescription to consumers + Coordinates pharmaceutical inventories, supplies. + Coordinates with registered pharmacists to meet agency needs + Maintain doctor schedule to ensure clients are seen regularly + Perform vitals on clients seeing the doctor and complete AIMS at admission and every 6 months + Complete PAP on clients as necessary + Prepare pillboxes daily, weekly, bi-weekly, monthly as appropriate + Assist psychiatrists as needed while they are seeing clients.
Work closely with doctor to assure medication refills are up date + Provide medication education to staff/ clients as needed + Case Management + Provide transportation for clients to access community resources + Emergency services/ on call duty on rotation that may include commitment procedures, after hour assessments, crisis planning, and hospital diversion + Travel to community to see clients and provide needed assistance Requirements + Must Complete all Initial & Required Annual Training (CQI, CPR, First Aid, Ethics, ect.) + Must have at least 2 years of experience working with individuals with severe and persistent mental illness. + Prefer QP status or be an Advanced Practice Nurse (APN) + Prefer Bachelor or Graduate degree in nursing. + Valid Drivers License Benefits + Medical + Dental + Vision + Life Insurance + 401K with company match + Paid Days Off + Tuition Reimbursement and much more!
