CONSULTING DC LICENSED TEAM LEADER (LICSW) - Intake Clinician

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:
Reports to the Clinical Director.
Completes intake services (Diagnostic Assessments, Person Centered Treatment Plans, GAINS, LOCUS, Crisis Plan) for consumers entering the Core Service Agency (CSA).
Intake clinician will also assist with providing individual therapy for PCC consumers.
Must be able to complete electronic treatment plans based on the self-identified needs of consumers; their Community Support Worker; psychiatrist, physician, nurses and other identified clinical service contacts assigned to the consumer. Familiarity with the Credible electronic system is preferred.
This position is with a Mental Health Core Service Agency Provider with the D.C. Department of Behavioral Health. This agency has created a range of supportive services and treatment which are designed to assist consumers in meeting their life goals and goals for personal recovery which are inclusive of:
Community Support Case Management
Psychiatric Services & Case Management
Medication Education and Management
Individual, group and family counseling
Support 24-hours a day to assist in times of emergencies
Supports & services designed to meet the needs of individual families
Dual diagnosis service groups and intensive linkages for a variety of addictions that are being experienced by consumers.
These services are utilized based on the results of the assessment and the Treatment Planning process. All consumers are served by their own team, which consists of a Team Case Manager, Psychiatrist, Nurse and Community Support Specialist. Other members of a consumer's team will be determined by his/her needs such as physical health care providers; vocational counselors; residential providers, substance abuse counselor, etc.
POSITION QUALIFICATIONS -
Requires Current DC Social Work License: LICSW.
- Must have excellent writing skills to enable the development of writing treatment plans and diagnostic assessments.
- Has excellent observation , verbal and written communication skills
- Demonstrates the ability to build effective teams capable of delivery quality services.
- Ability to deal tactfully with internal and external customers.
