Provide clinical and administrative leadership and oversight to meet and promote quality standards and contribute to the organization mission and philosophy.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:
Administrative Responsibilities:
1.) Coordinate the Quality Assessment, Performance Improvement (QAPI) program for the organization.
2.) Coordinate the provision of ongoing clinical education to the hospice staff.
3.) Assist in the orientation/education of newly hired clinical staff.
4.) Manage and enter clinical data into the SHP (Strategic Healthcare Programs) system, present routine reports to leadership/staff as requested and assist in developing improvement projects as indicated.
5.) Provide education to the hospice team on the CAHPS Hospice Survey and report findings on a routine basis.
6.) Develop and assist in the administration of an ‘Integrative Therapy Department’ to provide complementary and alternative therapies as part of Hygieia’s clinical hospice services.
7.) Develop and assist in the administration of the ‘We Honor Veterans Program’ (WHV) through NHPCO, including participation in a Hospice-Veteran Partnership and education to both hospice staff and the community.
8.) Review Hygieia Hospice materials developed to educate the community to assure compliance with the mission of the organization.
Clinical Responsibilities:
1.) Serve in the role of ‘office nurse’ when requested.
2.) Evaluate and report if HIS form (Admission Assessment), captures appropriate clinical information.
3.) Provide guidance and oversight transitioning from Kinnser to Hospice Engine EMR.
4.) Participate in training and education of staff on Hospice Engine EMR.
5.) Development and staff education of We Honor Vets Program (WHV)
6.) Develop and present clinical in-service programs for Hygieia Interdisciplinary Team.
7.) Develop and facilitate annual skills lab for the clinical team.
8.) Conduct field performance evaluations for clinical nursing staff including Certified Home Health Aides (CHHA).
9.) Perform medical record reviews using an audit tool to ensure regulatory and Joint Commission compliance with Conditions of Participation (CoPs) and standards of care.
Job Requirements:
1. Current/active Registered Nurse license, BSN and CHPN preferred
2. At least 5 years of hospice leadership or in-kind experience
3. Working knowledge of Federal/State Hospice Regulations/ Conditions of Participation (CoPs)
