Role Summary:

Every day, we care for patients with chronic conditions in their home. We try our best to keep them feeling well and comfortable so that they can live their lives to the fullest. But life can be tough, which makes it hard for people to stay healthy and get better when they are sick. Patients have worries such as: How will I get to the pharmacy to get my prescriptions filled? and Will I have food, transportation and enough money?

As the CommunityConnect Operations Director, the goal is to support the team to build a bridge between the patient's clinical and social well-being.

The CommunityConnect Operations Director is a full-time, experienced, community health services professional, with responsibility for directing the operations of the team and driving results of a new care delivery program in California for California Advancing and Innovating Medi-Cal (CalAIM). The Director is a leadership role to ensure our quadruple aim objectives are met which will require operational performance oversight, targeted interventions, and process improvement to create enduring performance.

This individual will have demonstrated throughout their career an unparalleled dedication and success in achieving greater health equity for vulnerable populations. The supervisor will bring an ability to act as a cross-functional leader, both through influence and collaboration to develop, advance, implement, and sustain program strategy and goals.

Role Responsibilities:

  • Works with business leadership to set goals for utilization, growth, operational and quality metrics, and oversee these metrics.
  • Identify opportunities for process improvements and staff education to produce superior clinical and operational outcomes
  • Provide leadership to the team.
  • Work with market and corporate leadership to improve key program functions including member outreach and engagement, CHW training and management, analytics, customer reporting and customer management
  • Tactical management of day-to-day activities including CHW capacity, visit rates, quality and utilization results and other tactical operational processes
  • Actively participates in interviewing and hiring of the supervision team
  • Provides coaching and mentorship to a multi-disciplinary care team towards success in their jobs
  • Makes territory assignments in collaboration with CHW Supervisors
  • Communicates key performance indicators (KPIs) to direct reports and holds direct reports accountable for KPIs
  • Conducts regular team meeting with his/her direct reports
  • Communicate closely with other community leaders
  • Completes performance evaluations for all direct reports
  • Other duties as assigned
Qualifications

Role Qualifications:

We are searching for a special breed of health care professional who embodies the following qualities and characteristics: heart and commitment to serve vulnerable populations, passion, and perseverance to achieve long-term goals (a.k.a. grit), team-based and social determinants of health orientation, and embrace change in a rapidly evolving health care delivery system. Flexible and dynamic, this self-starting individual will be a creative problem solver with a proven track record of successful implementation of innovative health care delivery solutions. They must possess excellent time management and organizational skills, with the ability to prioritize and multi-task. Additional qualifications include-

  • Local to California (remote)
  • Bachelors Degree in Business, Finance, Healthcare, Social Work or related
  • Five or more years of direct management experience preferred
  • Direct experience managing large strategic customers and complex programs
  • Preferred experience with Medicaid
  • Communication Skills
  • Must have a strong desire to build close relationships with all members of the field and regional teams.
  • Must be committed to working in a collaborative relationship with the members primary care provider.
  • Excellent time management, organizational skills, and strong communication is a must.
  • A creative problem solver with a proven track record of leading and building cross-functional project teams to drive measurable results
  • Strong organizational skills and multitasking abilities with an ability to operate effectively in a high-growth, high-performance culture
  • Demonstrated ability to perform multiple concurrent tasks with minimal supervision and meet deadlines
  • Flexibility and fluidity in managing many responsibilities
  • Strong analytical, quantitative and operational skills
  • Contributes positively among the care team and actively promotes teamwork
  • Ability to navigate and use an electronic health record for reporting, and working knowledge of computers and ability to document effectively and efficiently
  • Drivers license requirements: licensed for a minimum of 3 years without restrictions; no DUI or other felony driving conviction in the past 7 years
  • Some travel required
