Pinnacle Partners has partnered with a reputable and growing organization in search of an experienced Client Care Manager. This role is specific to assisting with in person visits to older adult clientele and their families to identify and coordinate personalized services offered by the organization. This position will involve daily travel, predominantly within the Indianapolis area and primarily occurring between the hours of 8am to 5pm.
The primary goal of this position is to create connections with clients and their families. This role will advocate for the client, and identify resources to meet the client’s needs. The ideal candidate will be compassionate, organized and resourceful - with excellent communication skills.
- Complete client assessments and educate the client and families about the service offerings
- Assist with the implementation of the service offerings, keeping in mind cost and budget
- Facilitate introductions between clients and care giver staff
- Collect feedback in order to stay in touch with the satisfaction of the provided care
- Visit and speak to everyone involved in the care process on a quarterly basis
- Report needs and feedback to Director level staff
- Problem solve through proactive behavior
- Assist with scheduling of care givers as needed
- Provide on call support one night per week, every other week
Qualifications:
- 3 to 5 years of experience in quality assurance, client retention and customer service management required
- Prior experience with social work, case management and or senior living industries is required
- Excellent communication skills
- Strong critical thinking and analytical skills
- Ability to adapt to changing business needs, challenges and goals
- Experience with metrics, reporting tools and processes
- Bachelor's Degree is required
