Job Description

We are seeking Contingent Licensed Practical Nurse LPN to work at our Madison Heights 7am-7pm Days and 7pm-7am Midnight. As well as our Dearborn Location to join our team! You will be responsible for delivering high quality care to assigned patients.

​Responsibilities:

  • Responsible to monitor clients through the withdrawal process and encourage treatment readiness.

    Specific Tasks:

    · Monitor the medical status of all persons served (via vitals, monitor medications, periodic observations of persons served, special attention to detox withdrawal symptoms).

    · Provide medical and nutritional didactics/groups to the persons served.

    · Conduct individual health assessments for each woman that enters STR, with a nursing care plan developed (day shift LPN primary responsible).

  • Observe & monitor the client’s vital signs, physical and emotional status using the CIWA assessment form.
  • Facilitate didactic groups with clients related to drug use, recovery, and other relapse prevention strategies.
  • Observe & monitor the client’s vital signs, physical and emotional status using the CIWA assessment form.
  • Monitor client in self-administration of medications for relief of symptoms. Maintain accurate medication log.
  • Engage client in conversations regarding treatment readiness, hopefulness, and strategies to achieve and maintain a clean, sober lifestyle.
  • Network with and become familiar with other local agencies and services that might be of service to clients during and after social detox placement.
  • Responsible to document all behaviors, physical symptoms and observations of clients during shifts, according to clinical protocols, including critical incidents.
  • Develop rapport and a professional therapeutic relationship with clients.
  • Completes phone intakes, gathering all relevant information to ensure client is eligible and appropriate for admission to the detox program.
  • Facilitate client-centered, engaging intake process. Coordinate client’s arrival to detox program; provide program orientation, establish therapeutic relationship, and gather initial information and paperwork from the client.
  • Responsible for knowing and adhering to the health and safety policies. Assist in maintaining a clean, sanitary and safe environment for clients by taking the initiative in household cleaning, meal preparation/clean-up, laundry and minor household repair.
  • Ensure all essential detox/STR functions are maintained in the absence of other team members including, but not limited to, coordination of client placement, client meals, ensuring safe, clean environment, phone coverage, essential data entry and basic housekeeping/laundry.
  • Perform all other related and/or assigned tasks by management.

Qualifications:

  • Previous experience in nursing or other medical fields
  • Familiarity with medical tools and equipment
  • Ability to build rapport with patients
  • Compassionate and caring demeanor
  • Excellent written and verbal communication skills
  • Experience with using EMR systems and computer skills
