Job Description

Due to the recent COVID-19 outbreak, all counseling services and treatment will be provided over the phone or via telemedicine until further notice. Our agency specializes in providing the convenient in-home counseling appointments. After telemedicine services have ended, the position will be solely in-home counseling. All employees are issued equipment to conduct telemedicine services, including a laptop and cell phone.

The Clinician is responsible for providing mental health assessment, treatment planning, and counseling intervention services, as well as continually monitoring and documenting patient progress and outcomes. The Clinician functions as a member of an interdisciplinary team, coordinating care and reporting patient progress and wellbeing to the clinical team and involved parties in accordance with the patient’s individualized treatment plan.

 

Required: Bachelor’s Degree in Behavioral Health or related field, Arizona Board of Behavioral Health Licensed, Valid Fingerprint Clearance Card, Employed with People Empowering People. TB test Annually, Required Trainings, valid drivers license with clean driving record, be 21 years of age or older, current Professional Liability Insurance, and auto insurance.

 

Reports to: Behavioral Health Professional, CEO, or assigned Supervisor.

 

Supervision: The Clinician shall receive supervision from an approved supervisor weekly and ongoing clinical oversight. The supervision and clinical oversight modalities include but are not limited to: individual face-to-face supervision, telephone, videoconference, or group. The supervisor shall also observe the Clinician providing treatment and evaluation services to patients through the use of face-to-face settings, by videoconference, by teleconference, or by review of audio or video recordings. Patients will be provided with appropriate written notice of clinical supervision.

 

Guidelines: The Clinician must Follow: All Federal and State applicable ways, People Empowering People Policies and Procedures, Arizona Board of Behavioral Health standards, Regional Behavioral Health Authority (RHBA), American Counseling Association, and ethics codes.

 

Scope: The Clinician provides services to patients within the home, office or community as agreed upon. The Clinician shall utilize clinically approved modalities, including but not limited to: the use of Dialectical Behavioral Therapy (DBT), Eye Movement Desensitization Reprocessing (EMDR), Cognitive Behavioral Therapy (CBT), Solution Focused, Brief Solution, and Focused therapies in accordance with their training and certifications.

 

Essential Functions:

 

1. The Clinician will provide counseling services based on mental health, psychological and human development theories, principles, and techniques to adults, families, children, and groups. Interventions will be provided primarily in person through the use of verbal or non-verbal communications with patients and involved parties, as indicated by the patient.

a. The Clinician will facilitate human development and adjustment; assess and facilitate career development; treat interpersonal relationship issues and nervous, mental and emotional disorders that are cognitive, affective or behavioral, and manage symptoms of mental illness.

b. Assess, appraise, evaluate, diagnose and treat a wide range of patients through the use of psychotherapy. A.R.S 32-3251.

c. Clinician will provide psycho education to patients, providing information regarding mental health, emotional disorders or behavioral health. ARS 32-325

 

2. The Clinician will provide indirect services to patients including but not limited to training, coordination of care, and clinical supervision. The Clinical I will work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with the patient, their family (as defined by the patient), behavioral health, medical and dental providers, community resources and others to ensure that all services prescribed in the individualized service plan are implemented.

a. Provides supportive services, including but not limited to, the following: Assistance in maintaining, monitoring and modifying covered behavioral health services;

b. Brief telephone or face-to-face interactions with the patient, family or other involved party for the purpose of maintaining or enhancing a patient’s functioning;

c. Assist in finding necessary resources other than covered services to meet the patient’s basic needs;

a. Serves as a point of contact and to ensure ongoing collaboration including the communication of appropriate clinical information with other involved parties as appropriate and coordination of care with a patient’s family, behavioral and general medical and dental heath care providers, community resources and other involved supports including educational, social, judicial, community and other state agencies; Ensures the provision of all covered services identified on the services plan;

b. Referrals to community resources, as appropriate, and coordination of care activities related to continuity of care between levels of care across multiple providers, services and supports;

c. Provides outreach and follow-up of services including, but not limited to, crisis and missed appointments to ensure adequate resources are available and in place;

d. Participates in staffings, case conferences, or other meetings with or without the patient or the patient’s family participating; when appropriate and not Case Managed by a PNO;

e. Ensures that transfers out-of-area, out-of-state or to an Arizona Long Term Care system (ALTCS) contractor, are coordinated as applicable;

f. Ensures the development and implementation of transition, discharge and aftercare plans prior to discontinuation of behavioral health services.

 

When appropriate: performs all case management functions associated with caseload including completing the assessment and services planning processes; including identifying the need for further or specialty evaluations. May sign off on service plans and annual updates completed for people on their own caseload. The Clinician collaborates with the patient and his/her family or significant others to implement an effective service plan, explaining the available clinical options to the team, including the advantages and disadvantages of each option. Maintains the patient's comprehensive clinical record, including documentation of activities performed as part of the service delivery process (E. G., Assessments, provisions of service, coordination of care, discharge planning). Provides continuous evaluation of the effectiveness of treatment through the ongoing assessment of the patient and input from the patient and relevant others resulting in modification to the service plan, as necessary. Other Duties As Assigned

 

Minimum requirements: the Clinician must meet criteria to be credentialed and privileged to conduct assessment, provide psychotherapy, and/or function as a clinical liaison; therefore, must maintain the following combinations of education and licensure.

1. Masters degree in behavioral health related field and able to meet the clinical needs of the population served as continually assessed by a licensed supervisor.

2. Must be licensed through the Arizona Board of Behavioral Health Examiners or be in the process of obtaining licensure.

 

 

Knowledge, skills, and abilities:

Computer skills including working knowledge of Microsoft Word, Excel, and Outlook. Experience with Apple products a plus. In addition effective interpersonal skills, ability to follow instructions, good written and verbal communication skills, time management skills, knowledge of psychiatric conditions, typing skills, and sound judgment.
