Penndel Mental Health Center is a non profit community based mental health agency located in Langhorne. We are seeking candidates to join our team of professionals who are passionate about working with individuals diagnosed with mental illness, intellectual disability, or drug and alcohol addiction. Under the direct supervision of the Team Leader, the Nurse is responsible for daily medication management and nursing support for consumers and staff. Minimum requirements include an RN degree and experience in either Mental Health or Medical-Surgical Nursing.
Provide Medication management for consumers:
Package medications on a daily basis for delivery by team members
Fill medication boxes on a weekly basis for consumers who require less supervision with their medications
Monitor new scripts – note all changes and new meds
Fax scripts to participating pharmacy
Assist psychiatrist and be available to answer questions
Interface with pharmacies and assist consumers with insurance issues
Medication education to consumers
Visit with consumers at home and in the community to provide injections, medication education, and wellness education
Blood pressure checks as needed
Complete nursing assessments for each consumer annually
Collaborate with mental health and medical providers to ensure continuity of care for consumers in program
