We are actively seeking a Field Therapist for our Forensics Program. Field Therapist provides individualized direct care services to an identified caseload of consumers with serious and persistent mental health disabilities. Field Therapist works in collaboration with consumers, family members, and other service providers to assist consumers in gaining access to needed services and community resources. The goal is the promotion of consumers’ recovery toward independence and community integration.
The responsibilities include maintaining a primary caseload of consumers, conducts assessments of consumer strengths and behavioral health needs, work collaboratively with consumer on caseload to develop individualized service plans based on assessments of strengths and needs and assisting consumers through community-based activities and as defined by individualized service plans.
This position requires a minimum of a Master’s degree in a human service field and a minimum of 2 years direct service with mental health consumer. It also requires a valid driver’s license and a criminal history, child abuse clearance and FBI clearance. Strong organizational skills and oral/written communication skills are needed.
