Hospice After Hours Nurse

Operates under the supervision of the after-hours supervisor and under physician orders. Responsible for the management and care of Partners In Care Hospice patients after business hours. The on-call nurse will be available by phone from 5PM-8:00AM Mondays through Fridays and for the 24 hour period beginning at 8AM on Saturdays and Sundays. S/he is responsible for identifying and coordinating patient/family care to support patients with advanced illness and families.

After Hours/On-Call Nurse occasionally may also be offered PRN shifts during regular business hours (Monday- Friday, 8am-5pm).

The Hospice nurse is required to provide point of service electronic charting and documentation at time of patient visit or admission. Ability to operate a computer and / or a tablet PC, and proficiency with Windows- based software programs, is required.

The On-call Nurse will provide teaching, assessment, and intervention skills to deliver comfort care and maximize the quality of life for the patients and families by:

  • Assume primary responsibility for Partners In Care Hospice total census during assigned shift
  • Receiving report from Clinical team.
  • Responding to calls received on behalf of the agency from after-hours phone line and voice mail system and follow-up according to agency policies and procedures.
  • Triaging patient related calls by determining the needs of the caller and then responding in an appropriate manner either by phone or with a visit.
  • Provide skilled nursing, intervention, and supportive care to patients in the home setting.
  • Educate primary caregivers, volunteers, and employed caregivers to provide care for patients as indicated.
  • Complete documentation within agency guidelines and to meet regulatory requirements. Submit completed documentation within 24 hours after on-call services are provided.
  • Accept referrals for new patients in accordance with the agency’s policies and procedures.
  • Communicate regularly with agency nursing staff to review problems or unique issues from on-call hours
  • Participate in the agency’s orientation and in-service training programs for professional staff
  • Perform specific assignments as directed
  • Assisting patients, family members or caregivers with concern and empathy; communicate in a courteous and respectful manner.
  • Coordinating services with contracted vendors, outside agencies and interdisciplinary team as needed.
  • Investigate and follow through on orders for medication or requests for service or information.
  • Attending patient deaths to provide support to the patient’s family, contact the mortuary, advise on disposal of narcotics, notify the IDG as well as contracted vendors providing services to the patient.
  • Responding to referrals calls, providing the caller with information regarding hospice care, screening calls for appropriateness, taking patient information and coordinating with the on call supervisor.
  • Reporting concerns to the hospice supervisor on-call.
  • performing other duties within scope of practice and as training permits.

JOB CONDITIONS

  • Position may be stressful.
  • On occasion, one may be required to bend, stoop, reach and move patient weight up to 250 pounds lift and/or carry up to 30 pounds.
  • Travel is required to and from patient's home when supervision of care is scheduled.
  • The ability to access patients’ homes which may not be routinely wheelchair accessible is required.
  • Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.
  • Must be available on the premises or via telecommunication during working operating hours.

EQUIPMENT OPERATION

  • Use of computer, telephone,tablet, calculator, copier and other office and medical equipment.

COMPANY INFORMATION

  • Has access to all patient medical records and patient financial accounts as well as personnel records of department staff.

QUALIFICATIONS

  • A registered nurse or licensed vocational nurse with two years of experience in a hospice or home health agency, primary care clinic or health facility.  (hospice preferred)
  • Must be a graduate of an accredited School of Nursing.
  • Must be a Registered Nurse (RN) or Licensed Vocational Nurse (LVN) licensed by the state.
  • Good communication skills to deal tactfully with Patients/Families and the community.
  • The ability to communicate well, both verbally and in writing.
  • Must have the ability to demonstrate self-confidence and positive attitude toward self and others and maintain commitment and enthusiasm to goal achievement.
