What is PACE North? PACE stands for Program of All Inclusive Care for the Elderly. We are new and unique option for individuals 55 and older who qualify for nursing facility level of care and wish to remain in their homes as they age. What does that mean? We care for the elderly in daily both our state-of-the-art day center on Garfield near Hammond during regular business hours and as needed in their home. Never heard of us before? That’s okay, we’re new to the region. We began serving the elderly in 2019 and our service model is so popular it’s growing quickly!

What does a RN Care Manager at PACE North do? You can expect to be responsible for the coordination of care for our participants in home, foster care or skilled nursing settings. You will attend our interdisciplinary team care planning and ensure the updated care for participants. Conducting assessments for our participants nursing and home care/safety needs, tracking and obtaining any medical supplies, equipment, products, ect that are needed for care planning, communicating with hospital discharge planners to minimize the participants length of stay and help facilitate a smooth transition back to the community setting!

What qualifications should you bring to the table? We’re looking for that special person who loves serving others with a positive approach. We’d appreciate at least 1 experience in an elderly patient setting and we would love at least two of recent experience working in home community health, have a CPR certification, but we can train here at the center. Valid license as a Registered nurse, Associates degree preferred, or a Bachelor’s of Science in Nursing, have a valid driver’s license, be medically cleared for communicable diseases and have recommended immunization up to date before engaging in direct contact with participants. With the driver’s license we also ask for valid auto insurance and reliable transportation.
