1. Maintain a caseload of 50-55 clients.
2. Assist and conduct intake interviews and assess clients in order to develop appropriate treatment plans.
3. Assist in the orientation of new clients to the program upon admission and ensure the dissemination of policies and procedures.
4. Develop and maintain up-to-date treatment plans for clients utilizing ASAM criteria standards. Review treatment plans monthly. Update annually.
5. Maintain Progress Notes on all contacts with clients.
6. Maintain continued stay reviews every 30-90 days pending on client progress.
7. Maintain discharge and continuing care documents.
8. Maintain monthly reviews for clients involved with TASC, DCFS, TANF, and Proviso etc.
9. Conduct regularly scheduled group counseling sessions.
10. Conduct Bi-weekly individual counseling sessions or more frequent as determined by the client’s need and service level expectations.(Level#1).
11. Conduct family sessions with the client; family members or significant other as needed to promote the client’s recovery environment/support system, and family education and support groups.
12. Maintain a referral system for clients and their family members/significant other. Provide advocacy, support and assistance to clients in obtaining needed indirect services and resources outside of PLCCA Inc. e.g. public aid, health care, shelter, vocational referrals, etc.
13. Ensure that DARTS and DARTS forms are completed and submitted to the assigned personnel in a timely manner.
14. Assist in obtaining and recording urine drops from clients, as scheduled, when needed with the Intake Manager.
15. Assist the Clinical Director with utilization reviews and quality assurance measures.
16. Become familiar with the Fee System and its impact on clients.
17. Attend all meetings, training sessions, seminars, etc, as directed by the Clinical Director.
18. Observe all S.A.O. RULES AND REGULATIONS, S.O.P.’s, DASA, FDA, and DEA.
19. Ensure the confidentiality rules are strictly maintained and that client records are adequately secured at all times.
20. Be aware of, and familiar with the Fire Drill Plan and Procedures, the Emergency Plan for Client Care, the Evacuation Plan, and the Alarm and be available and accessible in these emergency or routine matters.
21. Assist other staff members in maintaining a safe, clean and orderly work environment.
22. Assume the duties of the absent Case Manager/Counselor, when required, in order to maintain an adequate level of service.
23. Complete all administrative duties and functions as requested by and assigned by the Clinical Director.

SPECIAL NOTES: Due to the nature of this position all employees are expected to maintain a high degree of professionalism and code of ethic standards. It is further expected that employees be flexible in executing their duties, and be open to the culture of organizational change.
STAFF DEVELOPMENT: Employees are expected to increase and improve their level of competency and skills through continuing education/training, etc. As well as to upgrade their Counselors certification, when appropriate.
Qualifications: You must have and continually be able to maintain a CADC. 2 years’ experience in a methadone setting, preferred.
