Counselor/Therapist/Social Worker

Path Behavioral Health is seeking a Counselor/Therapist/Social Worker to Provide outpatient counseling/therapy to both SUD and Mental Health clients.
Primary Responsibilities and Duties
- Primary contact with client
- Agree to abide by federal and state regulations on confidentiality, as well as:
- Personnel policies and procedures
- Client abuse and neglect policy
- Client rights policy
- Client grievance policy
- Develop a standard plan and coordinate provision of services to meet the primary, urgent needs of consumers
- Provide intake and assessment services to referred prospective consumers
- Participate with supervisor to determine client needs, eligibility and appropriateness for treatment
- Development, implementation and revision of assigned consumers individualized treatment plan
- Review of treatment plan in accordance with agency policy
- Provision of individual counseling, group counseling, group education, treatment planning, discharge and aftercare planning
- Provision of ancillary treatment services as required (e.g. support or self-help groups, community referrals, special topic instruction, etc.)
- Counseling assistance to children and family members as appropriate and within scope of practice
- Maintenance of the clinical records in accordance agency policy and legal requirements
- Ensure timely documentation to support each consumer interaction and coordination of services as per agency policy
- Model ethical and professional standards of behavior at all times
Minimum Qualifications
- Perform each essential function satisfactorily
- Self motivated and able to make quality ethical decisions independent of supervisor when necessary
- Ability to research new and current information using available technologies
- Entry level knowledge of multiple modalities of treatment
- Prioritize and manage multiple projects simultaneously
- Possess basic computer skills including E-mail, Word, and Excel
Credentials and Academic Requirements
- BSW with LSW, MA Counseling, MA with LPC, MSW, MSW with LCSW, LSW or LGSW
- Master’s degree with licensure preferred

- Must obtain clearance with the WV Cares background check
- Valid Ohio/Kentucky/West Virginia drivers license indication individual is at least 21 years of age
- Personal automobile insurance

Must have the ability to present a positive image of the agency to the community. Must work cooperatively with other agencies in team atmosphere.
