Counselor/Therapist/Social Worker

Path Behavioral Health is seeking a Counselor/Therapist/Social Worker to Provide outpatient counseling/therapy to both SUD and Mental Health clients.

Primary Responsibilities and Duties

1. Primary contact with client
2. Agree to abide by federal and state regulations on confidentiality, as well as:
   1. Personnel policies and procedures
   2. Client abuse and neglect policy
   3. Client rights policy
   4. Client grievance policy
3. Develop a standard plan and coordinate provision of services to meet the primary, urgent needs of consumers
4. Provide intake and assessment services to referred prospective consumers
5. Participate with supervisor to determine client needs, eligibility and appropriateness for treatment
6. Development, implementation and revision of assigned consumers individualized treatment plan
7. Review of treatment plan in accordance with agency policy
8. Provision of individual counseling, group counseling, group education, treatment planning, discharge and aftercare planning
9. Provision of ancillary treatment services as required (e.g. support or self-help groups, community referrals, special topic instruction, etc.)
10. Counseling assistance to children and family members as appropriate and within scope of practice
11. Maintenance of the clinical records in accordance agency policy and legal requirements
12. Ensure timely documentation to support each consumer interaction and coordination of services as per agency policy
13. Model ethical and professional standards of behavior at all times

Minimum Qualifications

1. Perform each essential function satisfactorily
2. Self motivated and able to make quality ethical decisions independent of supervisor when necessary
3. Ability to research new and current information using available technologies
4. Entry level knowledge of multiple modalities of treatment
5. Prioritize and manage multiple projects simultaneously
6. Possess basic computer skills including E-mail, Word, and Excel

Credentials and Academic Requirements

1. BSW with LSW, MA Counseling, MA with LPC, MSW, MSW with LCSW, LSW or LGSW
2. Master’s degree with licensure preferred

1. Must obtain clearance with the WV Cares  background check
2. Valid Ohio/Kentucky/West Virginia drivers license indication individual is at least 21 years of age
3. Personal automobile insurance
