Nurse Practitioner (House Calls)

We are looking for a Nurse Practitioner who dares to unbreak the system, restore purpose, meaning and guide care and health back into healthcare.

In this role, you will focus is on screening and preventive primary care services delivered in the home, community and nursing facility settings. You will work primarily in non-clinical settings and provide medical care to adult patients with a focus of home bound seniors. This position emphasizes a team-based approach to care. You will also perform comprehensive medical assessments, order appropriate tests and procedures, formulate treatment plans, obtain specialists' consultations as needed, and complete documentations as required.

Essential Functions:

  • Conduct comprehensive in-home health assessments for P3 members
  • Formulate a list of current and past medical conditions using clinical knowledge, judgment and current findings
  • Address both chronic and acute primary care complaints, and able to ascertain medical urgency
  • Order pertinent diagnostic laboratory and radiology; able to work within an environment of limited resources and therefore uses diagnostic tests judiciously and appropriately
  • Prescribe medications as appropriate
  • Communicate gaps in care to member's primary care provider
  • Identify medical issues that may warrant a referral to a specialist
  • Identify urgent and emergent situations for proper intervention
  • Provide post discharge coordination to reduce hospital readmission rates and emergency room utilization
  • Educate P3 members on topics related to health conditions and promotion of wellness
  • Must have the ability to travel to member's visit location
  • Proficient with electronic medical records and the internet
  • Ability to work independently with support from market clinical team
  • Accept feedback and direction from the supervising physición
  • Additionally, perform face-to-face synchronous video communications using TeleHealth platform based on business need and leadership direction
  • Ability to navigate stairs and a variety of dwelling conditions and configurations
  • Ability to sit, stand and kneel as needed to perform physical assessment
  • Can thrive in a fast-growing, mission-driven organization focused on using data and sound clinical judgment to improve patient outcomes
  • Must be detail oriented, dependable, and professional

    • Education and Experience:

  • Current unrestricted licensure or ability to obtain licensure as a Family/Gerontology/ Adult Nurse Practitioner in the state of Nevada
  • Completion of Nurse Practitioner program at the master's level, with a national certification from one of the following organizations: American Academy of Nurse Practitioners; American Nurses Credentialing Center
  • 3+ years of experience as a Nurse Practitioner in a setting of primary care, complex care, geriatric or palliative care experience
  • Board Eligible or Board Certified (BE/BC) in respective discipline
  • Current DEA license
  • Must meet credentialing criteria
  • Preferred experience in Home health as a licensed clinición, especially in management of chronic conditions
  • Strong communication skills with the geriatric or Medicare population
  • Fluency in a language in addition to English is plus
