Job Description

 

Open Door Family Medical Center provides health care and wellness programs to individuals and families in need throughout Westchester, Putnam and Ulster Counties. A pioneer Federally Qualified Health Center, Open Door provides more than 300,000 patient visits annually and serves nearly 60,000 individual patients who might not otherwise have access. Open Door offers integrated services and a holistic approach to building healthier communities. Primary medical care, dental care, integrated behavioral health care, clinical nutrition, wellness programs, and chronic disease management are the foundation of its clinical programs.

Founded as a free clinic in 1972, Open Door’s mission of building healthier communities through accessible, equitable, culturally competent health care has led to site and service expansions. Today, Open Door operates centers in Brewster, Mamaroneck, Mount Kisco, Ossining, Port Chester, and Sleepy Hollow, in addition to eight School-Based Health Centers in the Port Chester and Ossining School Districts, a mobile dental van in Mount Kisco, and a new dental practice in Saugerties, NY.

Open Door has partnered with CHCANYS in seeking AmeriCorps Members for its Healthy Corps Program.  Members are placed in the Ossining and Port Chester medical centers; Open Door has four types of positions available for the upcoming year:

(1) WELLNESS – Enroll patients in exercise classes; lead the Recess Rocks! ® Nutrition Matters! and Cooking Matters Grocery Store Tour programs in our Wellness Centers as well as in the local community (schools, libraries, community centers, etc.)

(2) EXTERNAL RELATIONS-BABY BOX – Member will make phone calls to patients to invite them to attend Baby Box Workshops; Make reminder phone calls; Make follow up phone calls to record program statistics; Help host Baby Box Workshops hosted in our health centers (3) BEHAVIORAL HEALTH – plan and facilitate groups and outreach activities to promote healthy development in children and family wellness; work in cross-disciplinary teams to develop innovative program and marketing approaches that reinforce behavioral health, physical health, nutrition, wellness and literacy messages and deliver this outreach primarily in the waiting rooms.

(4) OUTREACH AND ENROLLMENT- will expand the services of the Outreach and Enrollment department by following up with patients that are enrolled in a health insurance plan to see if they have any questions about its utilization and provide financial education regarding their plans.

Program Benefits: The benefits include an annual stipend of $14,000, education award upon successful completion of service, Health benefits, student loan forbearance, and professional development.

Service Areas: Children/Youth, Health, Education, Elder Care, Community and Economic Development, Community Outreach. Westchester County is a suburban area with relatively high living costs and limited public transportation.

Requirements:

·       A car is highly recommended.

·       General Skills , Medicine , Teaching/Tutoring , Communications , Education , First Aid, Public Health, Leadership , Community Organization , Social Services , Writing/Editing , Public Speaking, Counseling

·       Spanish Speakers preferred.
