+ Develops and maintains active working knowledge of Wraparound program requirements, processes and procedures.
+ Collaborates with the Intake Coordinator in creating, implementing and maintaining treatment targets and programming goals for clients funded by Milwaukee Wraparound.
+ Maintains treatment plans, participates in treatment review meetings, and completes quarterly treatment plan updates in conjunction with Behavioral Treatment Therapists and team for assigned caseload.
+ Monitors staff implementation of treatment goals and strategies by providing in-person oversight at least monthly and feedback to staff as necessary.
+ Trains and mentors Behavioral Treatment Therapists in the home on therapeutic approaches.
+ Prepares and maintains written case notes following company standards and program protocols. Reviews team members case notes and provides feedback as necessary.
+ Collects data on target behaviors and target programming to include in treatment plan updates and report to the team at meetings to determine effectiveness of treatment.
+ Provides each family the opportunity to learn the skills outlined in the client's treatment plan and goals by meeting with parents at least twice a month; once individually and once in collaboration with Behavioral Treatment Therapist.
+ Attends monthly team meetings in the home as scheduled by Wraparound Care Coordinator/team as required by the Wraparound program to collaborate and monitor treatment progress.
+ Directs programming to prescribe a comprehensive, person-centered treatment program of support for each family.
+ Identifies gaps in service provision and/or company procedures and communicates these issues with the appropriate supervisory staff.
+ Establishes and maintains positive collaborative relationships with staff, families, and funding sources.
+ Functions within legal, licensing, company, and professional standards in maintaining a therapeutic environment for all people served, and monitors protection of all rights and reports any violations according to company policies, state, and/or federal regulations.
+ Conducts self in positive, respectful, and collaborative manner, demonstrating proactive actions and decision making to ensure supportive work relationships with internal and external stakeholders.
+ Complies with, and positively reinforces with others, expectations outlined in organizational policies, administrative procedures, confidentiality requirements, and Employee Handbook.
+ Works schedule as determined by the needs of their caseload and under the direction of the their manager.
+ A Master's Degree in Human Services or related field required.
+ A minimum of two years of experience providing therapeutic support to special needs students strongly preferred.
+ One year of supervisory or mentoring experience preferred.
+ Demonstrated knowledge, skills, and experience supporting children, adolescents and/or young adults diagnosed with emotional, mental health, neurological and/or developmental disabilities.
+ Valid driver's license for at least 3 years, valid car insurance, and reliable transportationalid for at least 3 years.
