Oversight of the clinical field staff.
Reports to Branch Administrator
Ensures patient coverage by qualified professional nurses.
Provides guidance, support, and oversight to nursing staff regarding case management activities to achieve positive patient outcomes.
Ensures timely, accurate submission of all paperwork to support compliance, reimbursement, and coordination of QI.
Acts as a liaison with physicians, community resources and other referral sources to ensure quality patient service and promote program growth.
Make patient supervisory visits when necessary
BSN (or RN with significant home care experience)
Minimum 2 years home care or related experience
Knowledge of State, Medicare, Medicaid, and JCAHO guidelines
Valid Drivers License
Proficient in computer use-operations fully integrated with EMR
