The NKADD is seeking a qualified individual for the full-time position of Intellectual and Developmental Disability Case Manager. This Case Manager has responsibility for coordinating services for individuals who are supported through a Federal Program enabling them to remain independently in their home, with help. Populations served are individuals with physical, intellectual, and developmental disabilities ranging in age from young children to older adults. This position assists clients and their families/caregivers in maintaining a plan of care, connects them with resources, and makes regular monthly visits to monitor services ensuring clients’ needs are being met. This role includes time sensitive documentation, required reporting, and compliance with regulations - all with the goal of maintaining clients’ services in their own community. The Case Manager works closely with both community resources and Medicaid offices in determining program eligibility for client base. Four-year college or university degree in the field of Human Services such as Social Work, Psychology, Sociology, RN or related degree and at least 1 year of experience in the field of health and human services required. Both education and experience must be validated. Candidates must have and maintain a valid driver’s license, reliable transportation, and willingness and ability to complete in-home visits on a monthly basis. Must possess solid organization skills, strong analytical and problem-solving skills, as well as the ability to make sound decisions. Collaboration with colleagues, consumers, and partner agencies a must. Maintaining positive work relationships, as well as a professional demeanor is critical to success in this role. Understanding of software necessary; must have ability to use, and a comfort level with, laptop computers. Applicants must successfully pass a background check and pre-employment health screening (urine drug screen and TB test).
