Northeast Family Services is seeking dedicated licensed mental health professionals to deliver Mental Health and Substance Abuse services directly to individuals and families. The licensed mental health professional will be responsible for adhering to professional and legal requirements and principles as well as demonstrating sound professional judgment.
LICSW, LMHC, LMFT from the state of Rhode Island.
Essential duties and responsibilities include the following, other duties may be assigned:
• Completes assessment of individual to obtain the data base information and formulate the integrated summary.
•Creates individualized treatment plans with measurable and attainable goals.
•Communicate with collateral service providers and/or patient’s families to coordinate care as appropriate, with patient consent.
•Ensures that all treatment methodologies are evidence based and reflect the most recent research findings.
•Updates patient treatment plan as needed.
•Appropriately records the essential process and content of each patient or significant other contact.
•Maintains professional license in good standing
•Discusses progress toward goals with patient, family, and referral agencies as appropriate.
•Must have a current and valid mental health license
•CCDP-D, CCDP or dual licensure in Mental Health and Addictions encouraged to apply.
