Job Description

GENERAL SUMMARY

To provide clinical support to physicians and prescribers.

MINIMUM QUALIFICATIONS

A Bachelor's Degree in Human Services field, and two years direct care experience in behavioral healthcare settings, computer skills, and customer service skills.

ESSENTIAL JOB FUNCTIONS

1.0 Provides direct care to clients in outpatient setting.

1.1 Performs FARS/CFARS.

1.2 Performs and records results of PHQ-9, Columbia Risk Assessment and responds to results in clinically appropriate manner.

1.3 Completes Outcomes as appropriate.

1.4 Performs basic vital signs as requested by physician.

1.5 Completes or oversees completion of MAP application where appropriate.

2.0 Develops and maintains a written treatment plan for assigned clients.

2.1 Treatment plans are maintained in accordance with the Agency's protocol and procedures for frequency and timeliness for completion of this document.

2.2 Treatment plans are appropriately signed and properly credentialed (including obtaining physician signature).

2.3 Written treatment plans are developed with participation from the client, family and other professionals, and significant others as possible and reasonable.

3.0 Assumes primary responsibility for coordinating assigned client's care with physician.

3.1 Communicates essential information regarding client care to all members of the client's treatment team, both within and outside the NHTC continuum of care as requested by physician. 

3.5 When appropriate and assigned, takes vital signs before the medication management visit.

4.0 Assessment Services

4.1 Performs appropriate intake assessments as assigned or needed.

5.0 Prepares written documentation for each service conducted.

5.1 Written documentation for each service conducted is performed in accordance with the Agency's standards for service documentation in the client's medical record.

5.2 Medical records accurately reflect the nature of service activities provided, timeliness of services and outcome.

6.0 Provide crisis intervention services as required.

6.1 Timely and effective crisis interventions are provided in accordance with the programs' standards and protocol for conducting this activity.

6.2 Is responsible for requests from other service modalities, both within and outside the New Horizons of the Treasure Coast, Inc. continuum of care, for on-site or telephone assistance in dealing with emergencies as well as providing essential clinical information to any professionals responding to a crisis. 7.0 Performs other duties as requested.

7.1 Miscellaneous duties are performed according to instruction.
