Job Summary:

Under general direction of the Associate Manger, Clinical Operations, the person employed as a Patient Programs Coordinator works with Specialty Pharmacy teams to coordinate, monitor, and audit manufacturer programs and patient management programs. This position aides in sustaining a culture focused on quality patient-centered care while ensuring optimal pharmacy operations. The Patient Programs Coordinator is responsible for performing duties in accordance with manufacturer and patient program activities. This position will assist in ensuring compliance with the manufacturer contracts and the patient management program guidelines.

Duties of the Patient Programs Coordinator include assisting in the management and outreach of patients enrolled in any manufacturer programs, data entry of clinician prepared patient counseling information into pharmacy software, documentation of patient information into partnering systems electronic health records and auditing of program compliance. This individual may also be involved in additional projects and responsibilities outside of regular pharmacy duties. This position may require assistance in training of new and existing Lumicera employees, as it relates to manufacturer and patient management programs.

The Patient Programs Coordinator position requires an extreme attention to detail, strong communication and documentation skills, and flexibility in role of pharmacy duties based on seasonality of business needs. This position will assist and collaborate with various clinical pharmacy staff with regard to pharmacy technician responsibilities.

Essential Functions:

  • Responsible for patient outreach via outbound phone calls to new and existing Specialty Pharmacy patients.
  • Responsible for communicating with doctor’s offices, clinics, or infusion centers to coordinate medication shipments for patients.
  • Responsible for answering inbound calls from patients, prescribers, and other healthcare professionals as it relates to manufacturer programs, patient management programs, and other specialty pharmacy duties.
  • Development of written patient/provider communication in regards to program management (i.e. patient/provider letters, or fax templates).
  • Responsible for consistently adhering to established workflows for program management through internal reporting software (i.e. Qlikview, 3D reporting).
  • Participate in interdepartmental and intradepartmental meetings in regard to manufacturer or patient programs or initiatives as necessary.
  • Coordinate with Quality Team members to assist with patient profile audits to ensure compliance with pharmacy accreditation standards.
  • Develop and perform ongoing profile audits for compliance with manufacturers programs.
  • Perform periodic reporting of metrics around program performance.
  • Develop presentation materials for manufacturer quarterly business reviews.
  • Responsible for input of clinician-prepared clinical assessments and patient management programs into pharmacy software.
  • Assist clinical staff with transferring clinical patient documentation into Epic using pre-populated templates.
  • Assist with patient transitions; examples would be with new generic launches, formulary changes, or other medication transition periods.
  • Responsible for other duties as assigned by clinical pharmacists and leadership team to meet patient and/or business needs.

Requirements:

  • High School or GED required.
  • Certified CPhT or ExCPT preferred.
  • Bachelor’s degree or equivalent experience preferred.
  • Minimum two years pharmacy support experience or healthcare environment experience preferred.
  • At least 1 years’ experience and demonstration of proficiency in position at Lumicera or equivalent preferred.
  • Computer skills with proficiency in Microsoft Outlook, Word, Excel, and PowerPoint preferred.

Physical Demands

  • Work tasks are performed indoors. Temperature is moderate and controlled.
  • Must be able to sit for extended periods of time
  • Requires grasping, writing, standing, sitting, walking, repetitive motions, listening and hearing ability and visual acuity.
  • Talking and hearing occur continuously in the process of communicating with customers and other employees.
  • Vision occurs continuously with the most common visual functions being those of near vision and depth perception.
  • Requires manual dexterity to use and operate all necessary equipment.
  • All employees must maintain a neat, clean, and well-groomed appearance and dress in a manner appropriate in a professional environment.
